Description
(i)This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) Solicitation number 09-235-SOL-00004 is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. (iv) This acquisition is a Buy Indian Set Aside in accordance with 25 USC 47. The NAICS code is 541611 and the size standard is $6,500,000. (v) There is only one contract line item which is to coordinate all aspects of a Diabetes Conference. (vi) The contractor for the Diabetes Conference shall do the following: Coordinate all aspects of a one day (9:00am – 4:00pm) conference for American Indians/Alaska Natives in the Sacramento Area for June 20, 2009. Coordinate all vendors both Native artists and diabetes care companies. Create an interesting and inspiring agenda promoting the Indian Health Service Standards of Diabetes Care and Prevention Standards of Care (may be located on the National IHS website). Arrange and fund all flights, ground transportation, hotel accommodations and incidentals for out of town speakers/diabetes specialists and faculty. Handle all registration logistics: name badges, DVDs for first hundred, program guides for all, provide bags for participants to put materials into. The contractor will provide a facility that will include: A conference room space, adequate to set up theater style for 300 - 500 participants. Must have audio visual capability The following equipment will be required: 1 Podium 1 gooseneck microphone 1 wireless belt pack microphone 1 handheld microphone 1 10 ft. screen 1 LCD support package 1 LCD projector 1 sound cable for laptop 1 small sound system and CD player for fitness session 1 AV technician to help operate equipment and ensure seamless delivery Break out room to hold 20 participants plus fitness trainer to do physical exercise, power supply needed. Vendor Area for 100+ people. Need 25 display tables. Need power for 4. Vendor area must be in close proximity to main conference space to ensure adequate traffic. Lobby or Foyer area large enough to handle the registration process. Prefer a hotel location with a minimum of 50 Federal Government rate rooms and possibility of 100 more and the hotel should be close proximity to the conference, within ½ mile. (Our agreement will not guarantee sleeping rooms but will want a specific cut-off date for individuals making and guaranteeing their rooms). The Conference must be within 8 miles from 650 Capitol Mall. Topics to be presented by the speakers: Motivation and empowerment of Native people struggling with Self-Management of Diabetes Mellitus. Educational topics to assist the conference participants to take care of their diabetes and understand what that involves. The relationship between emotional health and diabetes. Developing a positive attitude toward overall wellness. The importance of foot care with diabetes. The importance of nutrition and diabetes. The importance of heart health and diabetes. The importance of kidney care and diabetes. Overall Native community wellness and diabetes prevention. Locate, schedule and fund various American Indian/Alaska Native speakers/diabetes specialists to provide information on the above described topics. Locate, schedule and fund nationally renowned American Indian/Alaska Native keynote speakers with a background in Wellness specifically focused toward Native people with diabetes. Locate, schedule and fund professional diabetes specialists with diabetes Case Management and experience working well with the Native American population and to provide one-on-one health screening and education to the conference participants. Topics to be covered at minimum: “Ask a Specialist” with a diabetes provider; MD, FNP or PA. Nutritional counseling with RD/CDE. Emotional counseling with PhD/Behavior Change Specialist. Foot Screening with a Podiatrist. Various health screenings: blood pressure, blood glucose and body fat testing with experience clinical staff. Provide an exercise component with professional fitness trainers. (vii) Delivery date is June 20, 2009. (viii) The provision 52.212-1, Instructions to Offerors—Commercial, applies to this acquisition. (ix) The provision 52.212-2, Evaluation—Commercial Items, applies to this acquisition. The following factors shall be used to evaluate offers: price, technical capability, and past performance. Technical and past performance, when combined, are significantly less important than price. (x) Offerors shall include a completed copy of the provision 52.212-3, Offeror Representations and Certifications—Commercial Items, with its offer. (xi) The clause 52.212-4, Contract Terms and Conditions—Commercial Items, applies to this acquisition. (xii) The clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders—Commercial Items, applies to this acquisition. (xiii) There are no additional contract requirements. (xiv) The Defense Priorities and Allocations System (DPAS) do not apply to this acquisition. (xv) Quotes are due by June 3, 3009.
