- ATTACHMENT 1 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. FA7012-09-T-0034. The solicitation document; incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-32. The North American Industry Classification System code is 315211 with a business size standard of 500 employees. This requirement is set-aside for 100% small business. LINE ITEM 0001 - 0024: Jacket, Men: Lightweight with Removable Liner Stock Number: 8405-01-298-6890 Manufacturer: Defense Supply Center Philadelphia; QUANTITY: 141 EA, UNIT PRICE_____________; EXTENDED PRICE____________. SEE ATTACHMENT 1 SPECIFICATION LIST FOR SIZES. INSPECTION CONDITIONS AND TERMS: Items shall be priced FOB Destination. Delivery/Acceptance will be at 50 Duncan Ave, Bldg 7100, Bolling AFB, Washington, DC 20032-0202. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and. 52.212-3, Offerors Representation and Certifications-Commercial Items are hereby incorporated. Addendum to FAR Clause 52.212-1 applies to this acquisition is as follows: 52.211-6, 52.252-1, 52.252-5: (b) DFARS (Chapter 2), and 252.225-7000. If proposing an "OR EQUAL" item, offerors must submit descriptive literature or a sample. The government shall not reimburse respondents for any costs associated with submission of the information being requested or reimburse expenses incurred to interested parties for responses to this solicitation. The government reserves the right to cancel this solicitation, either before or after the closing date. Offeror must include, with its offer, a completed copy of provision FAR 52.212-3 in full text. FAR 52.212-3 can be obtained from http://farsite.hill.af.mil or from http://orca.bpn.gov if registered in ORCA (Online Representation and Certifications Application). An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at ORCA. If an offeror has not completed the annual representations and certifications at the ORCA website, the offeror shall complete paragraphs (b) through (i) of this provision. EVALUATION PROCEDURES: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforms to the solicitation and price is determined most advantageous to the Government in accordance with the requirement. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. If offering an equal product, interested parties must submit clear and convincing descriptive literature demonstrating the capabilities to satisfy the requirement specified above. If additional information is required to support price reasonableness, each offeror may be required to provide a hard copy of their price break-down. Non-conformance with this requirement may result in the offerors quote being determined unacceptable. FAR CLAUSE 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, including the following sub-clauses, 52.219-6, 52.219-28, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-36, 52.222-50, 52.225-13, 52.232-33, 52.233-3 and 52.233-4 apply to this acquisition. To be eligible for an award, contractor must be registered in the Central Contractor Registration (CCR) database. Contractor can contact the CCR by calling 1-888-227-2423 or e-mail at http://www.ccr.gov. NO EXCEPTIONS. A DUNS (Dun and Bradstreet) number is required in order to register. All invoices shall be submitted through Wide Area Work Flow. You can register at https://wawf.eb.mil. All responses must be received by 12:00 P.M. Eastern Standard Time (EST) on 3 June 2009.
