- This is a copy of the RFQ/Solicitation #N00183-09-T-0272. Please respond on or before the closing date of 20 May 2009. If there are any questions, please contact Carolyn Johnson via phone (757-953-5740) or email (adline.johnson@med.navy.mil). This solicitation is restricted to small business. Requirement to be awarded on a brand name or equal basis. Item on this RFQ is a Hemodialysis Machine for the Nephrology Clinic at Naval Medical Center-Portsmouth manufactured by Fresenius USA, Inc. Quotes should reflect pricing on all items for consideration. Please provide government & destination pricing. Also, state warranty information for each CLIN number (Line Item). If freight charges apply, indicate if charges will be “per shipment” or “single one-time charge”. Contractor shall supply a current published price list or GSA/VA Federal Supply Schedule contract for supplies &/or services being procured. Refrigerated, frozen, or hazardous material must be indicated as such & a copy of the MSDS (Material Safety Data Sheet) must be supplied for any hazardous material. Specify any special handling instructions. Solicitation closes on Wednesday, 20 May 2009.
