- Combined Synopsis/Solicitation W81NWY91012600 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-29. This acquisition is solicited on a 100% Small Business Set-Aside for NAICS 339112, size standard 500 employees. All responsible sources may submit a quotation which shall be considered by the agency. The requirements of this solicitation are for Cerebral Tissue Oximeter (2 each) and sensor kits (5 kits /carton) for use with Oximeter, ordered on indefinite delivery indefinite quantity (IDIQ) for Brooke Army Medical Center, Fort Sam Houston TX 78234. Required IDIQ with FOB Destination, base year and 4 option years for period 1 June 2009 through 30 September 2013. Line item no. 0001: Fore-Sight Cerebral Oximeter, model MC-2010 with battery backup or equal, 2 each. Line item 0002: Fore-sight Dual (large) sensor kit carton (5 kits/carton) or equal; 32 cartons. Salient characteristics of line item 1: - Absolute monitor of cerebral tissue oxygen saturation to accurately determine absolute levels of brain tissue blood oxygen saturation and brain venous oxygen saturation, (SvO2) & SctO2%) (30% arterial blood and 70% venous blood) - For use with Large Fore-Sight sensor types - Non-invasive and continuous or spot-check capable - Society of Thoracic Surgeons (STS) data collection capable - Accurate and reliable measurements - Intuitive user interface with ability to store user preferences - Delivers accurate measurement in low or zero perfusion states - Because of being absolute, no baseline measurements are necessary - Instantaneous feedback enable swift response times The following FAR and DFARS provisions and clauses apply to this solicitation: 52.212-1 [Instructions to Offerors]; Addendum 52.212-1; Federal Acquisition Regulation (FAR) provision 52.211-6 Brand Name or Equal applies to this acquisition. Vendors shall comply with FAR provision 52.211-6 if submitting other than the brand name products. Award will be all or none.
