WAGE DETERMINATION 05-2103 (REV7) DOLRFQ473-0994-494 STATEMENT OF WORK (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Proposals are requested and a written solicitation will not be issued. (ii) This solicitation number is DOLRFQ473-0994-494 and is being issued as a Request for Quotation (RFQ). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. (iv) This acquisition is 100% set-aside for small business. The associated NAICS code is 238330 with a $14 million size standard. (v) Contractor shall submit a Firm Fixed Price quote for this requirement. This requirement is being procured Open Market, and is not an acquisition off vendor’s GSA Schedules. This requirement is for “BRAND NAME OR EQUAL”. FAR clause 52.211-6, Brand Name or Equal applies to this acquisition. If offering an “OR EQUAL” item, please provide all specifications in your quotation. A description of services is detailed in the attached Statement of Work. (vi) It is anticipated that delivery and installation will be completed over one to two weekends, between Friday evening May 15, 2009 through Sunday evening May 17, 2009 (and a second weekend if needed). Delivery of this acquisition is FOB destination. (vii) The following clauses and provisions are incorporated and will remain in full force in any resultant award: (full text of clauses and provisions may be accessed electronically at this website: http://farsite.hill.af.mil) FAR 52.212-1, Instructions to Offerors-Commercial (viii) FAR 52.212-2 Evaluation-Commercial Items Evaluation. Award to be made on the basis of Lowest Price Technically Acceptable (LPTA). (ix) Contractors shall include a completed copy of FAR 52.212-3, Offerors Representation and Certifications Commercial Item or complete electronic annual representations and certifications at http//orca.bpn.gov. (x) FAR 52.212-4 Contract Terms and Condition-Commercial Items applies to this procurement (xi) FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Item are incorporated by reference, however, for paragraphs (b) and (c) the following clauses apply; FAR 52.219-6, Notice of Total Small Business Set-Aside; 52.222-21, Prohibition of Segregated Facilities; 52.222-26, Equal Opportunity; 52.222-35, Equal Opportunity for Special Disabled Veterans, Veterans of the Vietnam Era, and Other Eligible Veterans; FAR 52.222-36, Affirmative Action for Workers with Disabilities; 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration. Requests should be marked with solicitation number DOLRFQ-473-0994-494. (xiv) A site visit is schedule for 10:00-11:00 AM, Thursday, May 7, 2009.
