Attachment C - Wage Determination Attachment B - Bid Schedule Attachment A - Performance Work Statement This is a combined synopsis/solicitation for a commercial service prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will NOT be issued. This is a Request For Quote (RFQ) and the solicitation number is F2V3F99097A001. This acquisition is 100% set aside for small businesses. The 6th Contracting Squadron, MacDill AFB, Florida, requires the following: environmental lab sampling. DESCRIPTION OF REQUIREMENT Approximately 80 various environmental samples for various locations on base. Please see Attachments: A) Performance Work Statement (PWS); B) Bid Schedule and C) Wage Determination for further detail. Pricing shall be submitted using the Bid Schedule format, to include a proposed scheduled date for all samples that are due. Dates should fall between June 1, 2009 and September 30, 2009. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-32 effective 31 March 2009 & FAC 2005-31 effective 20 April 2009, DFAR DCN 20090115 and AFFAR AFAC 2009-0318 effective Mar 18, 2009. The North American Industry Classification System code (NAICS) is 541380, Testing Laboratories. A firm fixed price contract will be awarded. All interested parties must bid on all items. BASIS FOR AWARD: Award will be made to the lowest price offeror that is determined to be technically acceptable. Award of a contract is contingent upon proof of all required certifications as detailed in Section 1.6.5 of the PWS. Copies of the required certifications shall be submitted with the proposal. DELIVERY ADDRESS: The service is to be procured on behalf of 6 Civil Engineering Squadron (6 CES), MacDill AFB (Tampa), FL 33621. The following Air Force Federal Acquisition Regulation Supplement (AFFARS) clause applies and is incorporated by reference: AFFARS 5352.242-9000, Access To Air Force Installations The following Air Force Federal Acquisition Regulation Supplement (AFFARS) clause applies and is incorporated in full texts: AFFARS 5352.201-9101, Ombudsman: (a) An ombudsman has been appointed to hear and facilitate the resolution of concerns from offerors, potential offerors, and others for this acquisition. When requested, the ombudsman will maintain strict confidentiality as to the source of the concern. The existence of the ombudsman does not affect the authority of the program manager, contracting officer, or source selection official. Further, the ombudsman does not participate in the evaluation of proposals, the source selection process, or the adjudication or protests or formal contract disputes. The ombudsman may refer the party to another official who can resolve the concern. (b) Before consulting with an ombudsman, interested parties must first address their concerns, issues, disagreements, and/or recommendations to the contracting officer for resolution. Consulting an ombudsman does not alter or postpone the timelines for any other processes (e.g., agency level bid protests, GAO bid protests, requests for debriefings, employee-employer actions, contests of OMB Circular A-76 competition performance decisions). (d) The ombudsman has no authority to render a decision that binds the agency.
