Description
Statement of Work This is a request for quotes under Federal Acquisition Regulation (FAR) part 8. Responses to this RFQ may result in a Single Award Blanket Purchase Agreement (BPA). REQUIREMENTS The VISN 10 Pathology &Laboratory Medicine Service of Department of Veteran Affairs Health Administration has a requirement for a consolidated source for reference testing for individual assays. Vendors interested in submitting quotations must meet the minimum requirement as listed below. 1.The contractor must be able to perform all the required tests for each facility listed in the following attachment 1 & 2 that complies with the College of American Pathology (CAP) and Joint Commission standards. 2.The contractor must be able to provide Pick-Up of patient specimens at each location Monday through Saturday. 3.The contractor must be able to report test results back to each facility in electronic format in a time frame that is commercial accepted for that test. 4.The referral lab should provide any/all special specimen collection containers - 72-hour collection containers, viral transport, DNA probe media, etc. 5.The vendor must provide a PC and interface to allow for electronic order entry and transmission to testing laboratory as well as result transmission and report printing at the referring site. Appropriate forms and requisition must also be supplied. Locations Contract performance will be required to perform assays as requested from the following locations: Wade Park - 10701 East Boulevard, Cleveland, OH 44106 Brecksville - 10000 Brecksville Road Bldg 2, Brecksville, OH 44141 Dayton - 4100 W. Third Street, Dayton, OH 45428 Columbus - 420 N James Road, Columbus, OH 43219 Chillicothe - 17273 St. Rt. 104, Chillicothe, OH 45601 Technical Questions All technical questions must be submitted in writing to Harvey J. McGowan, harvey.mcgowan@va.gov. by via e-mail only. All responses to questions that may affect offers will be incorporated into a written modification to the RFQ. Questions submitted after the dead line that may affect the competitive process will not be addressed. SUBMISSION REQUIREMENTS AND EVALUATION The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. 1.PRICING: Offerors shall provide a pricing schedule, catalog of tests offered, as a response to this RFQ and a copy of the Federal Supply Schedule (FSS) Authorized Price List. Offerors are requested to show the FSS price, list any discount counts against the FSS price, and an extended price to be made the same for each location. Prices should be made available to all locations after award of a VISN 10 FSS-BPA. 2.TECHNICAL CAPABILITY, QUALITY OF SERVICE, and SUPPORT TO BE PROVIDED: Offerors shall provide, in addition to the pricing schedule, a copy of the FSS Contract information description that supports the capabilities of the offeror to provide the aforementioned service for evaluation to include copies of the CLIAA certificates and certifications from any accrediting body. A copy of any current quality assurance/quality controls monitors and any reports from the last twelve months. The vendor will provide 24-hour customer support via telephone. The purchasing activity is not responsible for locating or securing any information that is not provided in your response. 3.PAST PERFORMANCE: Provide a list of other institutions in which the contractor has provided the same services or similar services in the last two years. Note: The Government may also obtain information from prior customers known to the Government, consumer protection organizations, and any other sources the may have useful and relevant information with regards to performance and history. The government will allow offerors to comment on any negative past perform information obtained by the government that may affect the award. To assist in this determination, offerors should furnish the following information: (i) Dates of contract (beginning and ending). (ii) Name and address of facility where services were rendered. (iii) Telephone number and name of contact person for each contract. (iv) Description of services and overall scope/size of the contract. 4.The ability to perform, at a minimum, the tests listed under "VISN 10 Contract: VISN 10: Pathology and Laboratory Medicine Services' Test List." 5.The ability to provide electronic invoices. Open Market Quotes and Items Open market items and services submitted in conjunction with items/services under a FSS contract may be accepted provided that the items/services are less then 50% of the total quote, are considered inseparable from the overall procurement, and are clearly identified. All FAR clauses applicable to open market items/services will be included in the order. Open market quotes not meeting the above criteria will not be accepted for this procurement. BASIS FOR AWARD The award of this solicitation shall be based on best value on factors other then lowest price alone. Award will be made to an offeror who is responsive and responsible. The award will be based on the offeror whose quote conforms the most to the requirements in the solicitation and is determined to be the most advantageous to the Government.
