Description
THIS IS A COMBINED SYNOPSIS / SOLICITATION NOTICE. This is a combined synopsis / solicitation for commercial items, prepared in accordance with the format of FAR 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written Request For Quotations (RFQ) will not be issued. The reference solicitation number is: NHLBI-PB(DE)-2009-136/DMR This solicitation is issued as a Request for Quotation (RFQ). This acquisition is being conducted under FAR Part 13, simplified acquisition procedures and the resultant purchase order will include all applicable provisions and clauses in effect through the Federal Acquisition Circular 05-24. The total contracted dollar amount, including options will not exceed $5 million. The acquisition is being conducted as a Sole Source. The North American Classification System (NAICS) code applicable to this requirement is 339999 and the associated small business size standard is 500 employees. The delivery point is the National Dental and Craniofacial Research (NIDCR), Veterinary Resource Core (VRC), Bethesda, Maryland. Market Research conducted by the Government has determined that the Lab Products, Inc.’s Hydropac Master Carton 4X system with 72,000 valves and 36 rolls of pouch film (BG9040302) will meet the Government’s requirement. The requested delivery date is 14 calendar days after receipt of the purchase order. The National Institutes of Health (NIH), National Heart, Lung, and Blood Institute (NHLBI), Office of Acquisition (OA), for the NIDCR, VRC, intend to award a single, fixed priced purchase order for the purchase of a hydropac Master Carton 4X system. The specifications of the Lab Product Inc.’s HydropacTM Master Carton 4X with 72,000 valves an Pouch Film (capacity to produce approximately 36 rolls of pouch film are as follows: the Master Carton provides materials to produce approximately 72,000 Hydropac Pouch and sterile Disposable ValveTM assembles. Each Master Carton consists of 36 rolls of Hydroseal Pouch Film (capacity to produce approximately 72,000 pouches) and 72,000 sterile (gamma radiated) disposable water valves. The sterile (gamma irradiated) Disposable ValveTM are packaged 100 per disposable plastic tray (60 trays per carton, 6,000 total per carton). The HydropocTM Master Carton consists of 12 separate cartons of disposable valves, 36 rolls of film and a thermoplastic base pallet for mobility. Materials to produce approximately 72,000 HydropacTM Pouches and sterile Disposable ValveTM assemble. Hydropac/Lab Products STERILE DISPOSABLE VALVETM The HydropacTM STERILE (gamma irradiated) Disposable ValveTM delivers water Pouches to laboratory rodents. This patent, single-use valve minimizes the potential for cross-contamination, conserves water and helps keep cages drier. Mice, rat’s hamsters and guinea pigs can readily access water from the valve. This unique design requires 2-grams of pressure; it allows access for water by weaning. Designed for use on HydropacTM Modular Diet Delivery Systems. The Disposable ValveTM can also be used with a wide variety of wire bar lids (adapter required). •Sterile (gamma radiated), single-use valve minimizes cross-contamination risk. •For mice, rats, hamsters and guinea pigs. •Weanling access water with only 2 grams of pressure. •Quick and easy installation. •100 per disposable tray. •Works with hydropacTM Modular Diet Delivery Systems. •Fits a wide variety of wire bar lid designs (with adapter). Hydropac/Lab Products HYDROSEALTM POUCH FILM HydrosealTM AWS-2500 Pouch Machine to form, fill and hermetically seal water pouches. Each 7-3/4” (196.9 mm) diameter roll yields approximately 2,000 individual pouches with 13 oz. (384 ml) Or 8oz. (236 ml) of water. HydrosealTM Pouch Film is specially formulated to provide exceptional sealing, flexibility, transparency and strength. Pouches are ergonomic to handle, allow easy visual inspection of water levels and store compactly. Dosing or sampling pouch contents is quick and easy with a Silcone Patch and syringe. For larger scale dosing an optional in-line Water Treatment Proportioner offers precise administering of additives directly into the source water. •Film roll yields approximately 2,000 water filled HydropacTM Pouches. •Specifically formulated for reliable performance. •Allows easy observations of pouch contents. •Pouches are ergonomic to handle and store compactly. •Allows dosing and sampling with Silicone Patch and syringe. •Compliant with US guidelines for direct food contact. •Film loads in minutes onto HydropacTM Pouch Machine. The provisions of FAR Clause 52.212-1, Instructions to Offerors-Commercial Items is applicable to this acquisition. The Offeror must include a completed copy of the provision of FAR Clause 52.212-3, Offeror Representations and Certifications - Commercial Items with its offer. The provisions of FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The addendum to FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, is as follows: The Offeror must include in its quote, the unit price, the list price, installation charges, shipping and handling cost, the delivery date, prompt payment discount terms, the F.O.B. point (Destination or Origin), and provide descriptive literature inclusive of specifications as part of the quotation. The DUN and Bradstreet Number (DUNs), the Taxpayer Identification Number (TIN), and the business size. The Offeror must be registered in the Government=s Central Contractor Registry (CCR) System, which is available at www.ccr.gov. The FAR Clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items - Deviation For Simplified Acquisitions, applies to this acquisition.
