- Wage determination for Minneapolis/St. Paul. RFQ clauses SF 18 Request for Quote. YOU MUST COMPLETE & SIGN THIS FORM WITH YOUR QUOTE. Statement of work required. This is a request for quotes for the installation of a digital video recording capability in four (4) interview rooms at the Lindbergh Terminal located in Minneapolis, MN. Each vendor who submits a quote must show proof of authorization/qualifications to perform work at the Minneapolis St. Paul Airport. The Service Contract Act [52.222-41] and Department of Labor Wage Determination 05-2287 (Rev. -6) apply to this order and are hereby incorporated by reference. The likely labor categories are in the 23000 occupation codes. Award will be made on the basis of lowest-priced and technically acceptable. The vendor who is determined to have the lowest price and who meets the requirements outlined in the Statement of Work will be selected for award. Awardee MUST register in Central Contractor Registration (www.ccr.gov) and receive payment via Electronic Funds Transfer (EFT) as a condition of the contract. Registration in CCR requires a valid Dun & Bradstreet number. Vendors do not have to register in CCR just to provide a quote. Registration MUST be complete before award can be made. YOU MUST COMPLETE AND SIGN Blocks 12, 13, 14, 15 and 16 on page one of the SF 18 form. Quotes that are incomplete, unsigned, or late will not be considered. All quotes must be RECEIVED by the Contract Specialist no later than April 30, 2009, 12:00 NOON Eastern Standard Time (Indianapolis).
