- Request for Quote 2009-Q-11255 The Centers for Disease Control and Prevention (CDC) intends to procure a service agreement in order to maintain the operational capability of specified laboratory equipment, as indicated in the attached Request for Quote, under simplified acquisition proceedures. The North American Industry Classification (NAICS) Code is 334516, and the Size Standard is 500 employess for this requirement. Delivery to: 1600 Clifton Road, NE, Bldg. 17 and 18, Atlanta, GA 30333. Period of Performance: 1year from award. THIS IS A COMBINED SYNOPSIS/SOLICITATION FOR COMMERCIAL ITEMS. THIS ANNOUNCEMENT CONSTITUTES THE SOLICITATION; A SEPERATE WRITTEN SOLICITATION WILL NOT BE ISSUED. QUOTES ARE BEING REQUESTED. The following provision(s) apply to this acquisition: 52.212-4 Terms and Conditions - Commercial Items (Oct 2008) applies to this acquisition; 52.232-1 Payments; 52.243-1 Changes - Fixed Price (Aug 1987); 52.243-1 Alternate II Changes-Fixed Price (Apr 1984); 52.204-7 Central Contractor Registration applies to this acquisition. Contractor must be CCR Registered prior to submitting a bid. The following FAR Clauses are hereby added to this clause: 52.247-35 F.O.B. Destination. OFFERORS SHALL INCLUDE A COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFERORS REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS, WITH YOUR QUOTE. The best way to complete this certification is to download the FAR Clauses (Volume II) at http://www.arnet.gov/far, print out the clause 52.212-3, Offeror Representation and Certification, and fill in the appropriate blanks.
