- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W44DVQ90937702 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32. This acquisition is unrestricted under NAICS code 334510, Size STD 500. The requirements in this solicitation: 1. HYDROTHERM ABLATION SYSTEM (M00656000K0/D) Qty 1 2. HTA PROCEDURE SET (M0056020-05) QTY 2 MFG: BOSTON SCIENTIFIC CORPORATION Vendors offering other than the brand name product shall meet or exceed the following Salient Charteristics: The Hydrotherm Ablation device was developed by BEI Medical Systems. This device allows the application of themo-destructive energy to the endometrium by free circulating saline heated to 90 degrees Centigrate, for a treatment time of 10 minutes. The fluid flows into the uterine cavity through an insulated shealth surrounding a 3mm (rather than the usual 4mm) hysteroscope. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other FAR clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 30 days after receipt of order, shipping shall be FOB destination to Reynolds Army Community Hospital, FORT SILL, OK 73503. Acceptance Location: Reynolds Army Community Hospital, BLDG 4300 Thomas Road, Fort Sill, OK 73503. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Evaluation of Quotes: Offers will be evaluated according to Far 52.212-2.
