- SOL: VA-261-09-RQ-0176 Due: May 16, 2009 Request for Quote (RFQ) -Online Medical Journal Subscription. This will be a sole-sourceprocurement for the Library, VA Northern California Health Care System,due to the determination that the proposed vendor is the mosteconomical, the sole-source for the items and/or the most efficientprovider of all required order items in total. The NAICS code is519130, size standard of 500 employees applies. Product Service code ofU099 applies. The actual solicitation should be posted on or aboutApril 15, 2009. Telephone requests will NOT be accepted. All questionsmust be submitted in writing, questions may be faxed to W. Ulibarri,(650) 849-0332 or emailed to bill.ulibarri@va.gov. You may also mail toWilliam Ulibarri, Consolidated Contracting Activity (CCA/90), VAPAHCS,Room C-140, Bldg. 6, 3801 Miranda Ave., Palo Alto, CA 94304. This is aone time procurement, delivery is requested as soon as possible.Electronic solicitation may be accessed at no cost at www.fedbizopps.gov. Any amendment issued against this solicitation will also be availablefrom the same URL addresses. Contractor is responsible for submittingall documents required in accordance with the solicitation, includingany amendments which will be posted on this website. Solicitation isalso subject to availability of funding. Price includes all parts andlabor to repair any failure due to a manufacturer or installationdefect; and one site visit during the warranty period for service orrepair. Quotations shall be submitted on an all or nothing basis.Partial quotations will be rejected. Quotations shall be valid for 60days from offer due date. This solicitation and incorporated provisions and clauses are those ineffect through FAC 2005-31. Offeror shall submit all information withoffer as required in 52.212-1; a completed copy of provisions 52.212-3Offeror Representations and Certifications.
