- The purchase order resulting from this Request for Quotations (RFQ) is for the monitoring and inspection services to be performed on the Lubinsky Coal Refuse Fire emergency project in Norwegian Township, Schuylkill County, PA. The project site is located approximately 800 feet northeast (in the woods) of 596 E. Bacon St, Pottsville, PA 17901. The work under this purchase order will consist of providing approximately 360 hours of monitoring and inspection services, 250 photographs and 20 hours of video for the construction work contracted for and performed at the Lubinsky project site. RFQs are being mailed to OSM's pre-established inspectors list for mine fires in the area where the work is to be performed. In addition, the RFQ and attachments along with this synopsis have been posted on FedConnect which is linked to FedBizOpps. Quotes will be due on April 22, 2009. An original hard copy of the completed RFQ (Standard Form 18 Blocks 11, 13, 14, 15, and 16), AML Contractor Information Form and resume (in the format provide) or copy of pre-qualification letter issued by OSM will be due at the depository at 3 Parkway Center, Pittsburgh, PA 15220 by close of business on April 22, 2009. Electronic and/or faxed quotes will not be accepted. Contractors doing business with the Government are required to have a DUNs number, register in the Central Contractor Registration at ccr.gov data base and complete on line representations and certifications at orca.bpn.gov.
