- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation of Purchase Request # W68MX390906178 is issued as a request for quotations (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32 and DAC 20090115. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510. The Size Standard for small business is 500 employees. This RFQ is being issued as Unrestricted. These supplies are for use at Madigan Army Medical Center for a one time procurement action. All responsible Contractors shall provide an offer for the following Brand Name or Equal Products: Line Item 0001: Sonosite M-Turbo 1.2 Ultrasound 1 EA Line Item 0002: OB/GYN Color Package, M-Turbo 1 EA Line Item 0003: Application Software, Package for M-Turbo 1 EA Line Item 0004: Transducer Biopsy Compatible 1 EA Line Item 0005: 5-2 MHZ Transducer Biopsy compatible 1 EA Line Item 0006: Mobile Docking System Enhanced (MDSM) 1 EA Line Item 0007: Triple Transducer Connect w/Quick Disconnect 1 EA Delivery shall be FOB Destination. The following FAR clauses and provisions in their latest editions apply to this combined synopsis/solicitation.
