Description
STATEMENT OF WORK Contractor shall perform the following tasks during the Base Bid contract period and during all or any of the executed Option Years. 1.Contractor shall move any and all items (desks, file cabinets, chairs etc.) as outlined by the Contract Officer Technical Representative (COTR) or other identified representatives of the Cheyenne VA Medical Center. 2.Contractor shall report to COTR for identification of items to be moved, location of identified items and area in which items will be moved to. Contractor will be provided a move list to corroborate with COTR instructions. 3.Contractor will provide two (2) personnel to handle all moving jobs to ensure proper safety. The Cheyenne VAMC will notify contractor at least forty-eight (48) hours prior of move detail being requested. If a move detail has already been requested and scheduled the Cheyenne VAMC reserves the right to cancel move request and will provide a forty-eight (48) hour notification of cancellation. 4.The Cheyenne VAMC estimates (400) hours per year of scheduled work. Pay rate will be hourly and the schedule of these man hours will be determined by the Cheyenne VAMC using the notification guidelines set forth above. GENERAL NOTES Contractor is required to furnish the following items. 1.All tools needed to properly move a variety of items as instructed. These tools include, although are not limited to, pallet jacks, desk jacks, dolly, moving skates and straps. Moving Truck if required, usually only 20% of the time. If a moving truck is required to travel more than 15 miles a fuel surcharge will be allowed. 2.All tools to perform minor tear down and reconstruction of identified items to better facilitate the identified items to be moved. 3.All personnel protective equipment (PPE) (safety boots, gloves etc.) for movers to be worn when moving identified items. 4.All personnel are expected to have working knowledge of moving tools being utilized and knowledge of proper safety techniques. The Cheyenne VA Medical Center will relocate and stage large items via forklift that cannot be moved by personnel. Any items that need to be transported by vehicle will be loaded and unloaded by contractor. The Cheyenne VAMC will provide personnel to drive any needed forklift or vehicle.
