- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-09-T-0039. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18. The North American Industry Classification System Code (NAICS) for this procurement is 325412; Federal Supply Code (FSC) is 6550; Standard Industrial Classification is 2834 and the small business size standard is 750. This requirement is an unrestricted open market request. Dwight D. Eisenhower Army Medical Center (DDEAMC), Fort Gordon, GA has a requirement for the following: Item No. 0001: Seven hundred twenty (720) 18FFDG, 15 Milliecurie Isotopes To be delivered when called as needed. UNIT PRICE $ ________, TOTAL CLIN $ _________. The delivery terms will be for all material to be delivered as needed after receipt of order. The preferred shipping method is FOB destination. The Government anticipates awarding a single award for this requirement. Items will be delivered to: DDEAMC (W33BWP), Warehouse Property Book Officer, building 300 East Hospital Road, Fort Gordon, GA 30905-5650. The following FAR Clauses and provisions in their latest editions apply to this solicitation. FAR 52.212-1, Instructions to Offerors-Commercial; FAR 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition; FAR 52.212-5, Contract Terms and Conditions Required to Implement Statues or Executive Orders-Commercial Items to include the provisions of 52.225-1, 52.222-3, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-50, 52.225-1, 52.246-2, 52.232-18, FAR 52.232-33, Payment by Electronic Funds Transfer-Central Contractor Registration; DFAR Clause 252.232-7003, Electronic Submission of Payment Request (Jan 2004); and DFAR Clause 252.204-7004 Alt A, Required Central Contractor Registration Alternative A (NOV 2003); all of which apply to this solicitation.
