- Solicitation for Ophthalmology and Optometry Services for the Phoenix VA Health Care System. This procurement is conducted under the authority of 38 U.S.C. 8153 on an unrestricted basis. The North American Industry Classification System (NAICS) Code for this procurement is 621111, size standard of $10.0 Mil shall apply. The Contractor shall furnish clinical and operative staffing needs at the Carl T Hayden VA Medical Center. Contractor shall also provide surgical procedures at Contractor's off-site facility or facilities. Estimated contract performance period is August 1, 2009 through July 31, 2010 plus four (4) one-year Option Periods extending the contract annually until July 31, 2014, if exercised by the Government. The offeror is responsible for downloading all amendments and other documents from the FBO website without further notices from the Phoenix VA Health Care System, 650 E Indian School Road, Phoenix, AZ 85012-1892.
