- Past Performance Questionnaire (Attachment 2) Representations and Certifications (Attachment 1) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. A Request For Quotes (RFQ) is being requested and a written solicitation will not be issued. SOLICITATION #: F3G5GA9063A001 This combined synopsis/solicitation is issued as a Request For Quotes. Please include this Solicitation number on your RFQ. NAICS: 334220. Sic Code: 3663. This combined synopsis/solicitation is being issued as a 100% small business set aside. The small business size standard for NAICS 334220 is 750 EMPLOYEES. Nellis AFB, Las Vegas, NV 89191, desires to purchase Transmitter/Receiver and associated Boresight Equipment consisting of the following items: 1 each ST Transmitter, ST, 4450-4900MHz,2W with Internal Encryption, Video Only; 1 each MR Receiver, MR, 4.4-5.0GHz, with Internal Decryption, Video Only; 2 each Custom 10ft cables with RCA for TX & RX with internal Viewlock; 2 each 30" Dishes with L-Bracket Assembly; 2 each Antenna Feeds, 4.0-6.0GHz,30dB, Dual Port, Circular (RHCP/LHC); and 2 each Cables, LMR195, SMA(M)-SMA(M)3ft COAX. This is a firm fixed price type contract. Price of the items listed on the attached Commodity Listing should include handling, shipment, freight, and delivery to fob destination which is 26 WPS, 556 TES, Bldg # 718, Creech Air Force Base, Nevada 89018. Therefore, please do not list shipment or freight costs as a separate line item on your price quote. The requirement will be solicited and evaluated under FAR 12, Commercial Items and FAR 13.5 Simplified Acquisition Procedures.
