- Description
- The Minnesota Army National Guard, through the United States Property & Fiscal Office for Minnesota, has a requirement for On-Site Medical Surveillance Testing for approximately 275 employees. The actual testing will take place at the following two locations: Camp Ripley, Little Falls, MN and the Army Aviation Support Facility (AASF) at Holman Field, St. Paul, MN. The following types of test/surveillance are anticipated: Complete physical by Occupational Health Medical Provider (no Chiropractors), Lab work to include: CBC, UA, Chemistry panel with HDL and LDL, Lead, ZZP, Heavy Metals (Cadmium & Chromium)(Blood Only), Pulmonary Function Tests, Respiratory Clearance for respirators and Quantitative Fit Testing, Audiograms to include an electronic download from vendor to the Governments Defense Occupational and Environmental Readiness System-Hearing Conservation (DOEHRS-HC) System and Otoscopic Examination, Vital Signs to include Blood Pressure and Pulse, Two copies of Laboratory Results (one copy for the employee and one copy for the Government Medical Record), Electronic Versions of Heavy Metal and Respiratory Clearances. The estimated solicitation date is o/a 20 March 2009. The estimated proposal due date is o/a 10 April 2009. A firm fixed-price Indefinite Delivery/Indefinite Quantity contract is anticipated with a Base Year and Three Option Years to be exercised at the sole discretion of the Government. Evaluation criteria are past performance, capability, technical management and price. This procurement is set-aside for small business. The associated NAICS Code for this solicitation is 621511, the business size standard is $13.5 Million.
