Description
The Food and Drug Administration is soliciting for an agency that can provide one temporary full time Laboratory Worker/Dishwasher on site for the Northeast Regional Laboratory, located in Jamaica, New York with the following qualifications: Scope of Work - Laboratory Worker – Dish Washer General: This position is located at the Food and Drug Administration in Jamaica, New York. Duties include working in a laboratory cleaning, drying and sorting a large assortment of laboratory items such as glassware, instruments, and related items. A general knowledge of established work procedures for performing routine tasks that have a sequence of steps such as operating laboratory glassware washers and dryers. A variety of glassware types will be handled requiring the proper way to restock shelves to have clean glassware available for use in analysis. Specific: Glassware will be collected, as directed, from designated area on analysts’ benches or wash carts in wash room for washing usually by machine. Washing requires a series of consecutive pre-cleaning steps, e.g. checking for chips, cracks, burrs or other imperfections, removing and properly disposing of unusable glassware, ensuring appropriate storing in designated laboratory area(s). Machine washing requires the ability to operate dish washing machines by inserting baskets, adding the specified amount detergent and other cleaning solutions, to include setting timers to start the wash cycle. At the end of the wash/dry cycle, each washed item is to be visually checked for chips, burrs, waste residues, stains, detergent films, and other imperfections. Chipped or burred items are to be brought to the attention of the analyst or supervisor. Upon consultation with either the analyst or supervisor, waste residues, stains, and/or detergent films are to be re-cycled through dishwasher and once cleaned, safely stored in designated laboratory area(s). Physical Effort: Ability to lift and carry objects weighing from 5 to 14 kilograms (10 to 30 pounds). Working Conditions: Workrooms are noisy due to operation of equipment such as dishwashers and air compressors. Must wear protective clothing such as safety glasses, gloves and laboratory apron while working in washrooms. Safety glasses are required at all time while in the laboratory. (i) This is a combined/synopsis solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals/bids are being requested and a written solicitation will not be issued. (ii) The solicitation number is 09-222-SOL-00002. This solicitation is issued as a Request for Quote (RFQ). (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26 Effective February 17, 2009. (iv) This is a 100% set-aside for small business, firm fixed price requirement. The associated North American Industry Classification System (NAICS) Code is 561320 Temporary Help Services. (v) An official authorized to bind the Offeror must sign the terms and conditions of the offer. Offerors that fail to finish required representations, or reject the terms and conditions of the solicitation, may be excluded from consideration. FOB Point Destination. Contract type: Commercial Item - Firm fixed price. Simplified procurement procedures will be utilized. (vi) The period of performance is 52 weeks (40 hours per week, 30 minutes lunch). (vii) The provision at FAR 52.212-1 Instructions to Offerors – Commercial Items applies to this solicitation. The following agenda have been attached to this provision: None. (viii) The provision at FAR 52.212-2 Evaluation – Commercial Items is applicable to this solicitation. The specific evaluation criteria to be included in paragraph (1) of that provision are as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (i) technical capability of the item offered to meet the Government requirement; (ii) past performance (see FAR 15.304); (iii) price. Technical and past performance when combined, are equal when compared to price. The technical evaluation will be determination as to whether the proposed contractor is capable of completing the work. The government is not responsible for locating or securing any information, which is not identified in the proposal. To ensure sufficient information is available, vendors must furnish as part of their proposal all descriptive material (such as cuts, illustration, drawings, or other information) necessary for the government to determine whether the product meets the technical requirements. Past performance will be evaluated based on information provided by the offeror as to actual performance to industry or government agencies. The government will award a contract to the offeror whose offer conforms to this solicitation and will be most advantageous to the government, price and other factors considered. The government reserves the right to make an award without discussions. (ix) Offerors must include a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications – Commercial Items with its offer. (x) The clause at 52.212-4, Contract Terms and Conditions – Commercial Items, applies to this acquisition. The following agenda have been attached to the clause: None. (xi) The clause at FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders – Commercial Items, applies to this acquisition. The following additional FAR clauses cited in this clause are applicable: 52.233-3, 52.233-4, 52.203-6, 52.219-6, 52.219-8, 52.219-14, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52,222-35, 52.222-36, 52.222-37, 52.225-1, 52.225-3, 52.225-13, 52.232-33, 52.232-34, 52.222-41, 52.222-42, and 52.222-44. (xii) The clause at FAR 52.213-4, Terms and Conditions—Simplified Acquisitions (Other Than Commercial Items). CCR Requirement-Company must be registered on Central Contractor Registration (CCR) before an award could be made to them. If company is not registered in CCR, they may do so by going to CCR web site at http://www.ccr.gov. (xiii)The Defense Priorities and Allocations System (DPAS) and assigned rating are not applicable to this solicitation notice. (xiv) Deleted from original solicitation as per this amendment.
