Description
The Department of Veterans Affairs New Jersey Health Care System (VA NJHCS) intends to make multiple awards for firm fixed price, best value contract(s) for the period of April 1, 2009 through September 30, 2009 with one (1) one-year option to extend subject to the Government's discretion and subject to availability of funds. Services are to be provided at the East Orange Campus of the Department of Veterans Affairs New Jersey Health Care System located at 385 East Tremont Avenue, East Orange, New Jersey 07018. This procurement is being conducted under FAR Part 12, Acquisition of Commercial Items and FAR Part 13, Simplified Acquisition procedures. This procurement is set aside for 100% Service Disabled Veteran Owned Small Business. The applicable NAICS code is 621111 with size standard $10.0M. The solicitation will be available on or about March 5, 2009 with quotations due March 20, 2009. RFPs (Technical and Price Proposals) are to be either mailed, hand-carried or sent electronically as per instructions page attached. For information, contact: Marla Jacobson, Contract Specialist, Marla.Jacobson2@va.gov. All questions must be submitted by e-mail. Requirements are for both licensed physicians (surgeons) and certified imaging technicians to provide Ophthalmology Services during the regularly scheduled VA Clinic and Operating Room business hours. The services include physicians (surgeons) to provide various ophthalmology surgical procedures as well as to provide medical care in Clinics to veterans at its East Orange Campus location in New Jersey. The physician services are to be provided by board certified or board eligible (within six months of accredited residency) physicians in ophthalmology. Each physician must have a current unrestricted medical in the State of New Jersey and must be able to be privileged and credentialed by the NJHCS. Offerors may bid on all or any portion of the services listed within the Schedule of Supplies and/or Services and Prices/Costs. The specialty services required include all of the following: General Ophthalmology, Glaucoma, Cornea/Uveitis, Retina, Neuro-Ophthalmology, and Occuloplastics. Services to be provided by Retina specialist include but are not limited to: laser treatment, evaluate fluorescein retinal examinations, perform pars plana vitrectomies, retinal detachment repair, epiretinal membrane peel, retinal lasers, and intravitreal injections. Services to be provided by Glaucoma specialist include but are not limited to: trabeculectomy, glaucoma tube shunt, and glaucoma lasers. Services to be provided by Oculoplastics subspecialty include but are not limited to: blepharoplasty, ectropion/entropion repair, eyelid reconstruction, enucleation, evisceration, orbital surgeries such as orbital tumor excision and decompression. Additionally, On-call services are needed by one General Ophthalmologist during weekend, holiday, evening and nighttime hours. Currently each of the specialty clinics is scheduled for 4 to 6 clinic sessions per month and two to four Operating Room sessions per month. The services of qualified ophthalmic imaging technicians are needed to perform A Scans, B Scans and OTC Scans one full day per week at NJHCS East Orange Campus (9 to 10 sessions per month).
