- STATEMENT OF WORK:Telephone System for retrieving diagnostic testing results for the Jack C. Montgomery VA Medical Center (JCMVAMC) The contractor personnel shall be responsible for providing services in accordance with the following requirements of the contract. This statement of work and any applicable terms and conditions shall be included in any subcontracts. 1. The JCMVAMC is responsible for providing a telephone system for the healthcare provider to report diagnostic tests and other messages for retrieval at the patient's convenience. 2. The JCMVAMC agrees to make available to the contractor all information necessary to properly service the account. In return the contractor will provide the information necessary to utilize and monitor the service. 3. The contractor will provide the appropriate materials as needed to maintain the service. 4. All patient information and data provided to the contractor shall be kept confidential and shall not be disclosed to anyone outside of the contractor. The contractor agrees to protect this information consistent with the provisions of the Health Insurance Portability and Accountability Act (HIPAA) and sign documentation acknowledging that a Business Associate relationship exists between the parties. 5. The JCMVAMC and any person or persons it authorizes to utilize the contractor system agree to utilize the service for its intended use only. It will be the responsibility of JCMVAMC to assure patient agreement for information provided via the contractor system. The contractor and the contractor service will be held harmless for any message content left on the system by the JCMVAMC previous or current personnel. 6. JCMVAMC will provide the contractor the names of physicians who will use the contractor services at the date of this agreement. Updates will be provided to the contractor for physicians joining or leaving its organization. Such updates will include the dates joining or terminating. The JCMVAMC shall endeavor to provide such updates at least 30 days prior to the effective date. The contractor will revise billing consistent with the addition or deletion of a physician. In all cases, additions or deletions shall be for a full month. 7. The contractor will provide monthly utilization reports to the Contracting Officers Technical Representative (COTR). Such reports shall indicate the number of reports left, reports retrieved and total calls received by the contractor. These reports will be provided for all physicians regardless of whether they have used the contractor service or not. Additionally, the contractor shall report any information or advice the contractor believes shall be helpful for optimum use of its services to the COTR. 8. Fees by volume are inclusive of all physicians within the Muskogee Veterans Healthcare System. Approximately 50 providers will be utilizing the service.
