Description
"Amended" Presolicitation/Solicitation Notice VA-260-09-RQ-0141 for VISN 20 Home Ventilator Systems and Services The Veteran's Affairs (VA), VISN 20 Contracting Division has a requirement to provide to veterans in their place of residence, home clinical respiratory services associated with the delivery, set-up and servicing of ventilator and related equipment systems; perform preventive maintenance, repairs, pick-up of equipment; provide education/training to patient and caregiver; and provide rental of ventilator systems to include all parts, labor, and expandable material or supplies and travel costs within the United States in accordance with the specifications and in accordance with all current Joint Commission Accreditation (JCA), State, Federal and applicable Food and Drug Administration (FDA) standards. Contractor SHALL BE JCA ACCREDITED FOR HME/CLINICAL RESPIRATORY THERAPY SERVICES and shall maintain accreditation for the life of the contract and will be registered annually with the FDA. The contractor will have a Credentialed Respiratory Care Practitioner available, with a pager, and mobile phone, 24 hrs a day, 365 days a year. Contract requirements, together with evaluation criteria will be covered in detail in the solicitation and work statement. The VA intends to make one Firm-Fixed Price (FFP) type award. This requirement will be handled as a "Total Small Business Set-Aside" in accordance with FAR 19.502-2, enabling competition from all qualified small business sources. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. It is anticipated that the period of performance will be for one base year plus two one-year option periods. The Request for Quotation (RFQ) will only be available on the FedBizOpps website at http://www.fedbizopps.gov/. Hard copies of the solicitation will not be available. The website provides all downloading instructions to obtain a copy of the solicitation. All future information concerning the acquisition, including solicitation and amendments, will be distributed through the website. You may download the solicitation and fax your quote in at (360)256-0936. Prospective offerors are responsible for downloading the RFQ and all attachments. It is the offerror's responsibility to monitor FedBizOpps for the release of the solicitation and any amendments that may follow. Potential offerors making a written request must include an adequate justification for not obtaining the solicitation via the internet. Telephone requests for diskettes or other inquiries will not be honored. Proposals and Requests may be provided via email at dennis.riches@va.gov. Offerors are cautioned to include the Solicitation Number VA-260-09-RQ-0141 on all documentation submitted in response to this requirement. The North American Industry Code for this procurement is 621610 Home Healthcare Services, and the size standard is $13.5 million in Gross Revenue. To be eligible for award, offerors must be registered with the Central contractor Registry at www.ccr.gov. Additionally, Representations and Certifications must be completed at http://orca.bpn.gov.
