- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is FM442790210460; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-28, effective date Dec 12, 2008; Defense DCN 20081218, effective date 18 Dec 2008; and AFAC 2008-1217 effective date 17 Dec 2008. This acquisition will be a sole source to Medtronic; the North American Industry Classification System (NAICS) code is 339113. The business size standard is 500 employees. The Federal Supply Class (FSC) is 6515. The Standard Industrial Classification (SIC) is 5047. Travis AFB intends to award sole source purchase for 3 boxes of Medtronic Bone Morphogenic Protein-Autograft replacement Infuse Kits. We need 3 Large II kit 8.0cc, Manufacturer # 7510800. Medtronic is the only company that is able to provide the product needed by the procedure date of 6 February 2009. Award shall be made in the aggregate, all or none. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items (June 2008).
