- Description
- This project is a request for quotations to provide Medical Services at the Blackwell Civilian Conservation Center, located near Laona, Wisconsin. Contractors must be registered in the Central Contractor Registration Database located at http://www.ccr.gov and must have completed electronic representations and certifications on the ORCA database located at http://orca.bpn.gov to be considered responsive to this solicitation. Personnel performing the work must be licensed in the State of Wisconsin. All other requirements are included in the solicitation, which shall be posted on the FEDBIZOPPS website for downloading. The performance period is 365 calendar days, July 1, 2009 through June 30, 2010, for the base period with 4 option year periods. The NAICS code is 621111. The small business size standard is $10 million. This project is 100% Set-aside for Small Business. The estimated price range for the base year is between $100,000 and $250,000. Paper copies of the solicitation will not be issued and telephone requests or fax requests for the solicitation will not be accepted. All responsible sources may submit a quote which will be considered. Quotes can be submitted directly from the information provided in the solicitation package as posted.
