- REQUEST FOR PROPOSAL Licensed Mental Health Consultant At Sacramento Job Corps Necessary bidding documents and specifications may be obtained from the Purchasing Department, on or after December 29, 2008. Send an email request for bid package to the buyer. The Job Corps is located at: SACRAMENTO JOB CORPS CENTER 3100 MEADOWVIEW ROAD SACRAMENTO, CALIFORNIA 95832 Contact person: Jesse Rosario, Buyer (916) 394-0770, ext. 4323 Email Address: Rosario.Jesse@jobcorps.org Proposals must be received in the Purchasing Department to the attention of Jesse Rosario, Buyer, by 4 PM on January 19, 2009 at which time they will be opened and reviewed. The period of performance will be an average of twenty (20) hours per week through June 30, 2010, with the possibility of three (3) one-year extensions. It is anticipated that the Request for Proposal (RFP) shall become the final contractual document provided to the successful bidder with minor revisions incorporating appropriate aspects of the bidder’s proposal. Bidders should therefore review the format and clauses of the contract in order to prepare a response to provide MENTAL HEALTH services. The Sacramento Job Corps Center reserves the right to reject any or all proposals or waive formalities in the bidding process.
