Description
Statement of Work (SOW) for Service Contract. Contractor will provide DeWitt Healthcare Network and (outlying clinics) with the Contractors Premium service offering which combines scheduled preventative maintenance, delivery of scanner consumables and training with Basic on-site service including spare parts, labor and travel. Service is available as an upgrade to the existing standard Limited Warranty. Contractor will accept a contract that will provide service for a base year, plus two option years (for a total of three years service). Contractor will provide Technical Assistance through a toll free number. Tehnical Assistance will be available Monday-Friday (excluding holidays) 8 a.m. to 5 p.m. EST Under this service offering, DeWitt Healthcare Network will receive the following services and kits: Contractor will provided on-site, break-fix repair Each service maintenance program will include on-site break-fix repair of Customer scanner, which includes spare parts, labor and travel for service of verified hardware failures. After a Contractor or Technical Assistance Center (TAC) representative identifies a hardware problem, a service call will be opened and logged. Contractor will then authorize an Authorized Field Engineer (AFE) will be dispatched to the product location to perform repair service if there is no security, safety or physical requirements that would restrict access to the product. Contractor will also provide (Consumables Kits) Each service maintenance program will include automatic delivery of consumable kit(s) to the end-user site. The number of kits depends on scanner model. Note: The Consumable kits provided Customer during the maintenance period will contain quantities of parts required for the cleaning and maintenance care of the Product based on normal use. Preventative Maintenance Visits Contractor will provide an AFE that will schedule an on-site visit to thoroughly clean and replace any necessary consumable parts. Service will be provided during normal business hours at a time to be mutually agreed upon between the AFE and DeWitt within 2 weeks of initiation. User Training Contractor will provide the first Preventative Maintenance visit within 30 days of the contract award; AFE will come on-site to provide DeWitt with operator training on routine scanner maintenance, consumables replacement, scanner features and basic scanner operation and troubleshooting. 4-hour Response Time Option To better serve the needs of DeWitt Healthcare Network with business critical scanning applications, 4-hour Response Time will be available for Fujitsu scanners located in select metropolitan areas. Term The term of this service offering shall be one year from the date of contract award, and two additional years-in one year increments-if the options are exercised. Warranty Contractor warrants that all services will be performed in a manner that meets exceeds the professional standard for service. TECHNICAL EXHIBIT 1 DELIVERABLES (A deliverable is anything that can be physically delivered but may include non-physical things such as meetings.) All deliverables become the property of the United States Government. DELIVERABLE-1 PM Event, FREQUENCY 2 Per Year Per Machine, CONSIST OF Thoroughly clean and replace any necessary consumbable parts. DELIVERABLE-2 Kit Contents FREQUENCY As Needed CONSIST OF 3 Pick Rollers, 3 Brake Rollers, Cleaning Kit (Departmental and Low-Volume Cleaning Kits shall include F1 cleaner, cleaning paper, cleaning cloth, cleaning swabs, and instructions) TECHNICAL EXHIBIT 2 PERFORMANCE REQUIREMENTS SUMMARY The following is a list of the contractual requirements that will be verified as contractually compliant by government personnel. Each requirement includes a government determined performance standard and the resource that is used to perform the verification. Any functional area not listed was omitted because it did not have a requirement identified as critical. Surveillance of any functional area that does not have critical requirements will be accomplished through surveillance of the contractor quality program. 100% Inspection: One Hundred Percent Inspection is exactly what the title impliesinspecting a requirement every time it occurs. Customer Complaints: Customer complaints will be investigated to ensure compliance with the PRS prior to being transmitted to the contractor. Non-PRS Items: The Government will periodically evaluate all other items in the SOW. Performance Objective-1 On-site Break-fix Repair, Performance Standard Arrive within 4 hours of request and have repaired no later than next business day, Acceptable Quality Level Contractor shall ensure that technical representative is on time and is professional in delivering his/her services, Monitoring Method C.O.R. / Customer Complaints Performance Objective-2 User Training, Performance Standard Provide customer with operator training on routine scanner maintenance, consumables replacement, scanner features and basic scanner operation and trouble shouting, Acceptable Quality Level Only an Authorized Field Engineer or Authorized Technician shall provide this training to ensure AFE or AT will be able to answer questions, Monitoring Method C.O.R. / Customer Complaints, Performance Objective-3 Preventative Maintenance Visits, Performance Standard Thoroughly clean and replace any necessary consumable parts, Acceptable Quality Level Contractor shall perform PM at or above industry standards, Monitoring Method C.O.R. / Customer Complaints
