Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W911SF-09-T-0006 is issued under the Simplified Acquisition Procedures (SAP), Part 13.5. This Request for Quotations is for a firm fixed price, service contract for Mortuary Services located at Fort Benning, Georgia. This requirement will include a Base Period of 1-year that will commence on or about 1 January 2009 thru 31 December 2009, with four (4) additional one-year option periods in accordance with Army Regulation 638-2; all applicable Federal, State, and Local Laws, and all terms and conditions of the contract. The magnitude of this service is between $25,000 and $100,000. This acquisition is a small business set-aside under NAICS Code 812210. The scope of work requires the Contractor to provide the services, supplies, materials, labor, and transportation necessary to provide care of remains for deceased personnel in accordance with all applicable Federal, State, & Local Laws, and the terms, and conditions of the contract. The work encompasses professional services and requirements, caskets and shipping cases, transportation and hygienic practices, crematory procedures, and urns. In order to be eligible for contract award, the Contractors facility, where the preparation of the deceased and other services of this contract are performed, except for cremation services, shall be located within a twenty (20) mile radius of the Columbus Government Center located at 110 10th Street, Columbus, Muscogee County, Georgia. This is due to a strict requirement to comply with service response time, and Government inspections at the Contractors facility where mortuary services are actually performed. Funeral Homes that are determined to be in the competitive pool will be required to undergo and inspection by the Government prior to contract award (see attachment A of the RFQ). The following provisions and clauses apply to this acquisition: FAR 52-212-1, Instructions to Offerors Commercial Items; 52.212-2 Evaluation of Commercial Items; the evaluation criteria stated in paragraph (a) of the provision are as follows: The significant evaluation factors, in the relative order of importance are: 1) Technical Capability 2) Price; and 3) Past Performance. The contract award will be offered to the best value offer made to the Government, considering, technical capability, past performance and price. 52.212-3 Offeror Representations and Certifications Commercial Items; 52.212-4, Contract Terms and Conditions commercial Items and 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive orders Commercial Items. Offerors shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items with its offer. This provision may be obtained at http://www.arnet.gov/far. All responsible sources may submit an offer (see evaluation factors), including unit pricing electronically, via email, to Chelsey R.
