- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This soliciation, FA2517-08-T-6172, is being issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquistion Circular (FAC) 2005-02. This procurement is a 100% Small Business set-aside IAW FAR 19.502-2 (a). The NAICS code is 813110 and the small business size standard is $6.5 million. The following commercial items are requested in this solicitation: Performance of contract services as the Catholic Parish Coordinator at Peterson AFB, CO 80914, Base Chapel, Bldg 1410. Past performance questionnaires shall be distributed by offeror to individuals who can validate experience and quality of work completed. A minimum of three references are requested. No more than five are allowed. If past performance references are not available, a neutral rating will be given. With the submittal of the request for quote, references (to include names and contact numbers) shall be provided. Offerors cannot complete questionnaires on themselves. The following provisions apply to this acquisition: The provision at FAR 52.212-1, Instructions to Offerors--Commercial Items (Jun 2008) FAR 52.212-2, Evaluation-Commercial Items (Jan 1999) The evaluation criteria are, past performance and price, with past performance being more important than price. The provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items (Jun 2008), and the offeror must include a completed copy of this provision with their proposal (https://orca.bpn.gov). The clause at FAR 52.212-4, Contract Terms and Conditions-Commercial Items (Feb 2007); FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders--Commercial Items (Jun 2008). The Government reserves the right to award on a multiple award or an all or none basis.
