- This Amendment incorporates the following changes to this combined synopsis/solicitation PR-R1-09-10005: 1) The response date has been extended to December 22, 2008 at 3:00 pm EST. The reference to the response day and time in the instructions to offerors in the attached document titled "T&D RFQ" has also been changed to reflect this extension. All other requirements and attachments in this combined synopsis/solicitation remain the same. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitues the only solicitation; quotes are being requested and a written solicitation will not be issued. Any amendments to this Request for Quotes (RFQ) will be posted as an amendment to this synopsis/solicitation in FedBizOpps. Any relevant questions and answers pertinent to all interested parties will be posted to this synopsis/solicitation. This Request for Quote (RFQ) represents a Performance Statement of Work for removal, transportation and treatment, storage and/or disposal of hazardous material and emergency response to chemical and oil spills. The attached documents include the following for this requirement: 1. FAR 52.212-1 - Instructions to Offerors - Commercial Items 2. Addendum to FAR 52.212-1 (Includes additional Instructions to Offerors) 3. Evaluation Factors 4. FAR 52.212-3 - Offer Representation and Certification for Commercial Items 5. FAR 52.212-4 - Contract Terms and Conditions for Commercial Items 6. Addendum to FAR 52.212-4 (Includes Performance Work Statement and other required clauses) 7. FAR 52.212-5 - Contract Terms and Conditions Required to Implement Statutes or Executive Orders for Commercial Items (Sept 2008) 8. Attachment 1 UNIT PRICING FOR TRANSPORTATION AND DISPOSAL AND EMERGENCY RESPONSE SERVICES (In Excel Format) 9. Attachment 2 HAZARDOUS WASTE PICK UP SCENARIO (In Excel Format) 10. Attachment 3 Service Contract Act Wage Determination applicable to this solicitation
