- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This is issued as a Request for Quote (RFQ) SP4410-09-Q-3006. The purpose of this RFQ is to Polyfill 84 forklift tires that are mounted on 21 forklifts located at the Defense Reutilization and Marketing office at Port Hueneme, CA. The actual address is listed in Block 15 on the SF 1449 of the RFQ. This procurement is a total small business set-aside with the North American Industry Classification Systems (NAICS) Code of 441320. The resulting contract will be a firm-fixed priced contract. All quotes from eligible offerors shall include a completed FAR 52.212-3, Offerors Representations and Certifications - Commercial Items. The following provisions/clauses apply to this RFQ: The following clause under FAR applies: 52.204-7 Central Contractor Registration (APR 2008). IMPORTANT NOTICE: Central Contractor Registration (CCR) applies to all solicitations. Lack of registration in the CCR database will make an offeror/quoter INELIGIBLE FOR AWARD. To be eligible to receive an award from a Department of Defense (DoD) activity, you must be registered in the Central Contractor Registration (CCR) database. To register, access the CCR homepage go to www.ccr.gov or call 1-888-227-2423 regarding questions to the CCR. The clauses and provisions included in this RFQ by reference can be accessed electronically at these addresses: http://www.arnet.gov/far/ and http://farsite.hill.af.mil/. Contract award will be based upon the evaluation factors of price and conformance to the solicitation. Questions regarding this combined synopsis/solicitation RFQ must be submitted in writing via e-mail to sarah.rutkowski@dla.mil. The issue date for this RFQ is 26 Nov 2008, and the closing date is 4:30 p.m. Eastern Time on 02 Dec 2008. Submit complete RFQ packages along with a completed bid schedule and a completed FAR 52.212-3, Offerors Representations and Certifications - Commercial Items by e-mail to sarah.rutkowski@dla.mil or by facsimile to (269)961-4474. All eligible responsible sources may submit a quote which shall be considered for award.
