- This is a commercial item acquisition conducted in accordance with Federal Acquisition Regulation (FAR) Part 12. This acquisition is set forth to provide assistance to the VISN 5 VA Capitol Healthcare Network Comprehensive Nursing and Rehabilitation Centers (CNRC) in changing the culture from an institutional - centered model to a resident centered model by providing consulting and training services to staff. This will be accomplished by (1) transforming the current nursing home culture to a resident centered approach; (2) empowering front line workers to help address issues related to the delivery of customized, high quality care; (3) changing the environment to foster a homelike environment; and (4) linking transformation of the nursing home culture to quality indicators. This contract will be for a base item of twenty (20) months with the option to extend work to other facilities within the VISN 5 Network. This procurement will be set aside for small business, NAICS Code 561320, size standard $6.0M. The electronic solicitation may be downloaded from FEDBIZOPPS website. The offeror is responsible for downloading all amendments and other documents from the FEDBIZOPPS website without further notices from the Department of Veterans Affairs, VISN 5 VA Capitol Health Care Network. Proposals shall be submitted by December 5, 2008 at 2:00PM Local Time either by mail or hand carried. Fax/E-mail proposals shall not be accepted.
