Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ). This solicitation document and the incorporate FAR provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. This requirement is 100% set aside for small businesses - the applicable NAICS code is 621111 and the small business size standard is $9.0 million. The Wilmington VA Medical Center, 1601 Kirkwood Highway, Wilmington, DE 19805 is soliciting quotations from businesses that can provide the following service as specified in the Statement of Work: Contractor shall provide Dermatology examinations for eligible veteran beneficiaries as referred by the VA Medical Center, Wilmington, Delaware. Service shall be performed by a board certified Dermatologist. The Dermatologist shall possess all licenses as required by state, local, and federal entities. The Contractor must possess all licenses, permits, accreditations and certificates as required by law. The contract provider(s) must be credentialed by the Wilmington VAMC before seeing patients, a process that may take up to eight weeks. Any replacements or additional providers provided by the contractor must be credentialed. This contract allows in accordance with direction from the Wilmington VAMC the contractor to see patients at various WVAMC CBOCS, the main facility in Wilmington Delaware and in some cases in the contractors on facilities. This flexibility is necessary to insure that the VA and the contractor make quality care and required dermatology care easily accessible to our patients. The WVAMCs primary service areas include the three (3) counties in Delaware with the hospital located in Wilmington Delaware being located in New Castle County, the CBOC in Georgetown located in Sussex County and the CBOC in Dover located in Kent County. The New Jersey CBOCs are located as follows: Vineland CBOC is in Salem County, Ventnor CBOC is located in Atlantic County and the Cape May CBOC is located in Cape May County. The contractor will be required to work at any one of the CBOCs designated by the VA. The following provisions and clauses apply to this solicitation: 52,212-1, Instructions to Offerors - Commercial; 52.212-2, Evaluation - Commercial Items, past performance and price; 52.212-4, Contract Terms and Conditions, Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; 52.233-2 Service of Protest; 852.233-70 Protest Content; and 852.233-71, Alternative Protest Procedure. Award will be made to the contractor(s) whose proposals are determined to be most advantageous to the Government, price and other factors considered. Offerors are required to submit a completed copy of FAR 52.212-3, Offeror Representations and Certifications - - Commercial Items. Offerors may obtain copies of the referenced FAR and VAAR provisions at http://arnet.gov/far and http://farsite.hill.af.mil/vfvara.htm, respectively. Quotations must be received by 4:00pm local time on December 18, 2008. If an offeror submits a quotation by e-mail, it is the responsibility of the offeror to obtain confirmation from the contracting officer that the e-mailed quotation has been timely received.
