- COMBINED SYNOPSIS SOLICITATION IAW FAR 12.603 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This requirement number FM442783050205, is issued as a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-03 and Defense DCN 20050524. The solicitation/acquisition listed here is 100% small business set aside (or as applicable). The Federal Supply Class (FSC) is 6515 The associated Standard Industrial Classification (SIC) is 5047 The North American Industry Classification System is 423450 and the associated business size standard is 500 Employees. Note (1 & 2). This request for quotation consists of the following: Item 0001: ECG Machine to include the following components: MAC55-CAAAXX-AXXX MAC 5500 CLR US Govt Standard AHA 2017210-001 MAC 3500/MAC 5500 System Trolley 2027792-001 Kit memory card reader English 2027777-001 Kit MUSe secure digital upgrade English 2028776-0001 Kit MAC 5500 Applications Training. QTY: 2 Each. Item(s) shall be delivered to FOB point Travis AFB, CA no later than 30 days after receipt of order. The following provisions and or clauses apply to this acquisition. Offerors must comply with all instruction contained in FAR 52.212-1, Instructions to Offerors-Commercial. Note (3). FAR 52.212-2 Evaluation-Commercial Items, paragraph (a) to this provision is completed as follows: (Note 4). FAR 52.212-3, Offeror Representation and Certification-Commercial Items, a completed copy of this provision shall be submitted with the offer. Quotes without this provision will not be considered for award. Contractor must ensure annual reps and certs are current or complete them at http://www.bpn.gov/orca/. FAR 52.212-4, Contract Terms and Conditions-Commercial Items will be incorporated into any resultant Purchase Order by reference. FAR 52.212-5, Contract Terms and Condition Required to Implement Statues or Executive Orders-Commercial Items include clauses (b)(8), (11), (12), (13), (14), (15), (16), (17), (22) and (c) others that may apply. DFAR 252.212-7001, Contract Terms and Conditions (Dev). DFARS 252.225-7001 Buy American Act and Balance of Payments Program. DFARS 252.225-7002 Qualifying country sources as subcontractors. DFARS 252.204-7004 Alt A, Required Central Contractor Registration. Clauses may be accessed electronically in full text through http://farsite.hill.af.mil/VFFARA.HTM. Vendors must be actively registered with the Central Contractor Registration (CCR). The CCR website is http://www.ccr.gov. Contractors are asked to submit their TAX ID number with their proposal. Quotes may be transmitted by e-mail or FAX to (707) 424-2712 and are to be received at the 60th Contracting Squadron no later that 4:30 PST on November 21, 2008.
