Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulation (FAR), as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is issued as a request for quotation (RFQ). This solicitation document and the incorporate FAR provisions and clauses are those in effect through Federal Acquisition Circular 2005-27. This requirement is 100% set aside for small businesses - the applicable NAICS code is 441110 and the small business size standard is $28.5 million. The Philadelphia VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA 19104 is soliciting quotations from businesses who can provide one or both of the following items: (1) a full-size sedan (Buick LeSabre or comparable) suitable for use in a drivers' education/rehabilitation program for veterans with polytrauma; (2) a mini van (Chrysler Town & Country or comparable) suitable for use in a drivers' education/rehabilitation program for wheelchair users. Adaptive equipment for each of these vehicles must include: MPS Right Handle Hand Control; Menox Push and Pull Hand Control installed - left side; MPD Key Adapter; Steering devices such as spinner knob, steering quad type, steering spinner single post, steering spinner palm type, amputee type, tri pin type, DSI multi function spinner knot - (2 handles one button switch one with rocker switch); Large Convex Mirror (all view); Park Brake Extension MPD; Left Foot Gas Pedal; Turn Signal Cross Over MPS; Back-Up Sensor Back Tracker; Instructor Brake on Passenger Side; Instructor Rear View Mirror on Passenger Side; Instructor Horn and Kill Switch on Passenger Side; Student Driver Magnetic Holder and Plate Vehicle Identification; Digital Accelerator Ring ; Global Positioning System. The van must be equipped with modifications for wheelchair driver and 6 way power driver seat. Quotations may be provided for one or both vehicles. Quotations shall be valid for 60 days after offer due date. Prices quoted should be F.O.B. destination, which is the Philadelphia VA Medical Center, 3900 Woodland Avenue, Philadelphia, PA. Vendors may submit quotations for one or both of the required vehicles. Responses to this combined synopsis/solicitation should include a proposed pricing, a description of the vehicle being offered, proposed delivery date (which should be stated as days after receipt of order), and evidence that the vendor has supplied similar vehicles, outfitted with similar adaptive equipment, with the past three years (supply the name and contact information for clients who can confirm that they have purchased similar vehicles from the vendor in the past). The following provisions and clauses apply to this solicitation: 52,212-1, Instructions to Offerors - Commercial; 52.212-2, Evaluation - Commercial Items and the applicable evaluation criteria to be used in selecting a vendor shall be suitability of proposed vehicle for intended use, past performance and price; 52.212-4, Contract Terms and Conditions, Commercial Items; 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, including clauses incorporated by reference under section b(3), (5), (8), (11), (12), (13), (14), (15), (16), (18) and (24); 52.233-2 Service of Protest; 852.233-70 Protest Content; 852.233-71, Alternative Protest Procedure; 52.211-6 Brand Name or Equal; 852.211-77, Brand Name or Equal; and 852.270-4, Commercial Advertising. Award will be made to the contractor(s) whose proposals are determined to be most advantageous to the Government, price and other factors considered. Offerors are required to submit a completed copy of FAR 52.212-3, Offeror Representations and Certifications - - Commercial Items. Offerors may obtain copies of the referenced FAR and VAAR provisions at http://arnet.gov/far and http://farsite.hill.af.mil/vfvara.htm, respectively. Any questions regarding this solicitation must be e-mailed to the Contracting Officer at the address shown below. Quotations shall be submitted in hard copy to: Elizabeth Youse, Philadelphia VA Medical Center (90C), 3900 Woodland Avenue, Philadelphia, PA 19104 or by e-mail to Elizabeth.Youse@va.gov. Quotations must be received by 2:00pm local time on November 21, 2008. If an offeror submits a quotation by e-mail, it is the responsibility of the offeror to obtain confirmation from the contracting officer that the e-mailed quotation has been timely received.
