- Presolicitation Notice for Ophthalmology and Optometry Services for the Phoenix VA Health Care System. This procurement is conducted under the authority of 38 U.S.C. 8153 and is 100% set-aside for Veteran Owned Small Business (SDVOSB). The North American Industry Classification System (NAICS) Code for this procurement is 621111, size standard of $9.0 Mil shall apply. The Contractor shall furnish clinical and operative staffing needs at the Carl T Hayden VA Medical Center and provide surgical and laser procedures at Contractor facility. Estimated contract performance period is February 1, 2009 through September 30, 2009 plus four (4) one-year Option Periods extending the contract annually until September 30, 2013, if exercised by the Government. The tentative posting date for the solicitation is November 14, 2008, with offers being due January 7, 2009. The offeror is responsible for downloading all amendments and other documents from the FBO website without further notices from the Phoenix VA Health Care System, 650 E Indian School Road, Phoenix, AZ 85012-1892.
