Description
The VA Central Iowa Health Care System in the Des Moines, IA area has an urgent requirement for the following. 100 parking spots. The location must not be more than.5 mile from vicinity of VA Central Iowa Healthcare System, 3600 30th St., Des Moines, IA 50310-5885. This announcement replaces previous announcement va-263-09-rq-0021. This is a combined synopsis/ solicitation in accordance with Federal Acquisition Regulation (FAR) 12.6, and supplemental information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This combined synopsis/solicitation will result in a firm fixed price. The NACIS code is 812930. Simplified procurement procedures will be used for this acquisition per Federal Acquisition Regulation (FAR) Part 13. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-27. Only written requests received directly from the requester are acceptable. The following FAR Provisions and Clauses are applicable unless otherwise noted: 52.212-1, Instructions to Offerors-Commercial Items; 52.212-2, Evaluation - Commercial Items; Paragraph (a) insert: "award will be based on the technically acceptable low offer." ; 52.212-3, Offeror Representations and Certifications- Commercial; 52-212-4, Contract Terms and Conditions- Commercial Items; and 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders- Commercial Items; 52.217-09, Option to Extend the Term of the Contract; and 52.232-19. 1.0 Purpose: The purpose of this requirement is to provide an off-campus parking area for the VA Central Iowa Health Care System in the Des Moines, IA area. For the period of 12 months 2.0 General Requirements: 2.1 Location. The parking spaces shall be located within a half mile VA Central Iowa Health Care System, Des Moines Division located at 3600 30th St., Des Moines, IA 50310. The anticipated spaces are required on 1 December 2008. 2.2 Site Conditions. The Contractor shall provide an area that meets requirements for fire and life safety. The occupying agency must have access to the area 24 hours per day, 7 days per week, and 365 days per year. In addition the area should be well lit from dusk to dawn and kept free of debris. Snow and ice removal should occur from the parking lot and approaches before working hours (7:00 a.m. to 5:30 p.m. Mon- Fri) when ever possible. In the event of freezing rain and/or 2" or more of snow during working hours removal shall occur within one hour from onset. Chemicals or sand may be used to reduce safety hazards. 3.0 Insurance/Indemnification: The Contractor shall acquire, and present proof of liability insurance in an amount and coverage as to meet the insurance requirements of the jurisdiction in which the storage facility is located. In addition, the Contractor shall indemnify the Federal Government against all claims and/or suits for damage to property, personal injuries, and the loss of limb or life at the storage facility. The Contractor shall ensure that all insurance(s)/indemnification(s) remain in full force and effect during the entire life of the agreement (purchase order). 4.0 Inspection and Acceptance: The Contracting Officer or Contracting Officer's Technical Representative (COTR) reserves the right to inspect, or cause the inspection of, the storage facility, and the management thereof, to ensure compliance with the contract.
