Description
This is a combined synopsis/solicitation issued pursuant to Federal Acquisition Regulations Subpart 12.6, as supplemented with additional information included in this notice. The announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This acquisition is issued as a Request for Quotes (RFQ) and will be awarded utilizing Simplified Acquisition Procedures based on lowest price technically acceptable. This solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-20. The NAICS code for this solicitation is 562211 and the small business size standard is $11.5 million. This requirement is to provide services for transportation, disposal and documentation of medical and pathological waste for the National Institute on Aging, IRP. These services will be for specified locations in the Baltimore area. Specifically the first floor loading dock area of the Gerontology Research Center, NIA, 5600 Nathan Shock, and the MPW Room 2B207 on the second floor of the Biomedical Research Center, 251 Bayview Blvd all located on the grounds of Johns Hopkins Bayview Medical Center,Baltimore, MD. Pick up will occur twice weekly on Wednesday and Friday between the hours of 8:30 am and 4:00pm with additional pick ups done as needed. Contractor shall provide all Medical Pathological Waste (MPW) boxes, heavy duty biohazard bags as box liners and tape. The boxes must meet the DOT packaging requirements for transportation of medical and pathological waste. Both large and small size boxes will be provided. Medical/pathological waste service personnel should be trained in the proper handling of biohazards and infectious materials. This training should meet or exceed the OSHA training standards for biohazards. All packaging, transportation, disposal and documentation of medical and pathological waste must be in accordance with current Federal, State and Local regulations. Estimated usage for a one year period is 4100 boxes and will vary weekly. Please include two references, copies of permits and certificates for medical waste transport and disposal. The provisions at 52.212-1, Instructions to Offerors-Commercial Items, apply to this acquisition. Offeror shall include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items.
