Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. The U.S. Army Medical Command, Center for Health Care Contracting has a requirement for fresh frozen cadaver specimens for the Army Medical Department Combat Extremity Surgery Course. The resultant contract is subject to the availability of funds. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Document # W81K04-08-CESC-2001 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-27, Effective 17 Sep 2008 and DCN 20080915. This acquisition is unrestricted under NAICS code 621991 and the small business size standard is $10.0 Million. The anticipated delivery schedule will be once a month beginning 16-Oct-08 thru 16-Sep-09. A description of the requirement to include the delivery schedule is included as an attachment to this announcement. The requirements in this solicitation are: Contractors shall be American Association of Tissue Banks (AATB) certified. All specimens shall be safe and free of infectious disease. Serological Testing will be done by the vendor and all specimens shall be negative for HIV I/II, Hepatitis B and Hepatitis C. All items will be packaged to prevent damage during transport. All transportation, delivery, and disposal charges shall be included in the price. This includes origin to destination and return to origin. Contractor is responsible for appropriate disposition of all remains.
