Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This request is for all hardware, software, installation, training, and maintenance services for a consolidated T-metrics Automated Call Distribution (ACD) System at the Brain Allgood Community Hospital (BACH), Yongsan, Clinical Support Division in the Republic of Korea. Contracting Command Korea (CCK) / 411th Contracting Support Brigade (CSB) is issuing this RFQ to solicit quotes for the purpose of entering into a Purchase Order. It is the Governments intent to award a sole source purchase order to Nortel Government Solutions, Inc based on the authority under FAR 6.302-1, 10 U.S.C. 2304 (1), Only one responsible source and no other will satisfy agency requirements. If you are interested in responding to this requirement, you may participate by submitting separate technical and pricing quotes in accordance with this RFQ. Quotes shall include the following: (1) Detailed outline of proposed methods and techniques for completing the tasks listed in the Statement of Objectives.(2) Brief discussion of any anticipated problem areas, along with potential recommended approaches for their resolution.(3) Reference any past experience installing and maintaining the T-Metrics ACD System with other Government end users.(4) The Offeror must include a payment schedule with a price breakout for the base period (30 September 2008 29 September 2009) of all hardware, software, installation, travel expenses, training, and maintenance services. The offeror must also include a price breakout for the optional period of performance (30 September 2009 29 September 2010) for the extended hours of Software Subscription Service. (5) Your Quote must include the following Information:Tax identification number (TIN),Dun & Bradstreet Number (DUNS),Contact name and email address CONTRACT AWARD Contract award shall be made to the responsible Offeror whose offer, in conforming to this RFQ, provides an overall best value to the Government, technical evaluation factors, and price considered. SOLICITATION PROVISIONS AND CLAUSES (1)The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular. (2)The provision at 52.212-1, Instructions to Offerors Commercial applies to this RFQ.(3)Please include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications Commercial Items with this offer.(4)The clause at 52.212-4, Contract Terms and Conditions Commercial Items applies to this acquisition. (5)The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items applies to this acquisition; including 52.203-6, 52.222-50 Alt I, and 52.232-33.(6)The following DoD clauses (DFARS) apply to this solicitation: 252.212-7001, 252.209-7001, 252.209-7002, 252.209-7004, 252.212-7000, 252.225-7043, 252.232-7003, and 252.233-7001 (7)The following United States Forces Korea local clause applies to this solicitation: 5152.225-4020 INVITED CONTRACTOR OR TECHNICAL REPRESENTATIVE STATUS UNDER U.S. - REPUBLIC OF KOREA (ROK) QUESTIONS: If you have questions regarding this requirement, please submit your inquiries on or before 1:00pm Local Korea Time, Friday, 26 September 2008. Submission shall be via email to angela.dash@korea.army.mil. RFQ DUE DATE: The RFQ Due date for responses, both technical and price is on or before 1:00pm Local Korea Time, Sunday, 28 September 2008. Submission shall be via email to angela.dash@korea.army.mil STATEMENT OF OBJECTIVES - PATIENT APPOINTMENT SYSTEM Scope of Work. The Contractor shall address the methodology and approach for meeting the government's requirements as outlined in paragraph three of the SOO. The government will evaluate the completeness, thoroughness and understanding of the Contractor's proposal. All proposals shall be submitted in commonly-available Microsoft" Office formats. All proposals shall list the paragraph numbers of this SOO and how each will be addressed. Proposals may also include Executive Summaries and additional information.
