- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with the additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This requirement F3G3LB8225A002 is issued as a Request for Quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. The NAICS for this solicitation is 339920. The size standard for a small business under this NAICS code is 500 employees. This is a commercial request for 2 Gym Mats and 10 Mat repair kits. To furnish all labor, tools, parts, material transportation, and facilities to acquire: 2 Gym Mats and 10 Mat repair kits. Salient characteristics are as follows: CLIN 0001: Quantity: 2 Gym Mats on which it is easy to run and wrestle and that support mixed martial arts activities such as throws. Examples: Wrestling Mats, Mixed Martial Arts Mats, Gymnastics Mats, ets. Vynil Gym mats Color = Gray Size: Approximately 42ft W x 42ft L so that when the mats are taped together it forms one large mat approximately 42ft L x 84ft W. The mat must be between 2 to 2.25 inches thick. The thickness requirement is due to safetey reasons. 2 12ft straps per mat included "OR EQUAL" A Multicolor 10ft logo (see attached) is to be placed between the two mats so that when the mats are joined to form one big 42'x84' mat the logo is in the center. CLIN 0002: Quantity: 10 10 Mat repair kits Repair kits include: (1) patch at least 7/8"x7/8", (1) patch at least 1 3/8"x 1 3/8", (1) 36" reinforcement filament for repairing tears at edges "OR EQUAL"(2) Alcohol prep pads PLEASE INCLUDE DESCRIPTIVE LITERATURE, AND/OR COMPLETE SPECIFICATIONS IF PROVIDING AN "OR EQUAL." Also provide Part number and manufacturer of quoted items. 52.225-2 Buy American Certificate. As prescribed in 25.109(a), insert the following provision: BUY AMERICAN CERTIFICATE (June 2003) (a) The offeror certifies that each end product, except those listed in paragraph (b) of this provision, is a domestic end product and that the offeror has considered components of unknown origin to have been mined, produced, or manufactured outside the United States. The offeror shall list as foreign end products those end products manufactured in the United States that do not qualify as domestic end products. The terms "component," "domestic end product," "end product," "foreign end product," and "United States" are defined in the clause of this solicitation entitled "Buy American Act-Supplies." (b) Foreign End Products: Line Item No. Country of Origin [List as necessary] (c) The Government will evaluate offers in accordance with the policies and procedures of Part 25 of the Federal Acquisition Regulation.
