- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6, as supplemented with additional info included in this notice. This announcement constitutes the only solicitation. Proposals are being requested and a written solicitation will not be issued. The government intends to award a sole source to Synthes. The NAICS is 339112. acquisition is issued as a RFQ. The US Naval Hospital Guam, Agana Heights, GU has a requirement for the following medical/surgical equipment: CLIN 0001: Part Number 105.954, Synthes Small Battery Drive with 14.4V Battery Pack Set. Quantity: 1 Set; Unit Price: $__________, Total Price: $__________. Request shipping cost (via air freight) direct to Guam: $_________, Date of shipping: ______________. Item must be delivered within 30 days upon receipt of order. Reference provisions 52.212-1, Instructions to Offers – Commercial Items, apply to this acquisition. FAR 52.212-4, Contract Terms and Conditions – Commercial Items apply to this acquisition. Offerers need to include a complete copy of provisions at FAR 52.212-3, Offeror Representations and Certifications – Commercial Items with their offer (registered in CCR, DUNS number, Cage Code and Tax ID number).
