- Notice of Intent to award sole source contract. This is not a solicitation. The Department of Veterans Affairs Medical Center, Providence, Rhode Island (VAMC Providence) intends to negotiate and award a contract for medical equipment, the Selecta II SLT System (510(k): K004006), for carrying out Selective Laser Trabeculoplasty (SLT) procedures to American Medical Depot, Opa-Locka, FL, an authorized distributor of Lumenis Inc, Santa Clara CA. The authority for this action is FAR 6.302-1. Lumenis, Inc. is the sole source of the required equipment. This is a commercial acquisition under FAR 12.6 and 13.106. The items to be sole-sourced are: Selecta II, 110V - System (0640-940-01) including footswitch, safety spectacles/goggles, operator manuals, laser safety sign, in-service training, warranty on all parts and system; Lumenis 900 Beamsplitter (0638-181-01). The NAICS is 339112. The size standard is 500 employees. The FSC is 6540.
