- This procurement is for comprehensive annual eye exams and low vision evaluations for eligible veteran beneficiaries of the Department of Veterans Affairs Medical Center, Grand Junction, CO. Eye exams are to be performed by a Board Certified Optometrist who possesses a full and unrestricted license to practice in the State of Colorado. This is for the contract period beginning beginning November 1, 2008 through October 31, 2009 with four option years. This will be an IDIQ (Indefinite Delivery/Indefinite Quantity) Firm Fixed unit price contract. Past performance/demonstrated capability, quality assurance and personnel qualifications are the evluation factors and are of equal importance to price/cost. Contract award is subject to availability of funding. This procurement is 100% set-aside for small business. The NAICS is 621320 and the size standard is $6.5 million. Offerors are advised that they are responsible for obtaining amendments. Proposals are due by October 10, 2008 4:00pm Mountain Time.
