Description
The original combined synopsis/solicitation notice posted on Sep 15, 2008 is amended as follows: This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. Representations and Certifications-Offerors shall include a completed copy of provision FAR 52.212-3, Offer or Representations and Certifications-Commercial Items with their proposal. Reference FAR Clause 52.217-8 in the original notice, fill-in information is: within 30 calendar days before the contract expires. Reference FAR Clause 52.217-9 in the original notice, fill-in information is: within 30 calendar days prior to contract expiration, 60 calendar days and not to exceed 4 years. The following local clauses are added: Permits, Taxes, Licenses, Ordinances, and Regulations Contractor shall obtain all necessary permits, give all notices, pay all license fees and taxes, comply with all Federal, state, municipal, county, and local Board of Health ordinances, rules and regulations applicable to the business carried on under this contract, and are responsible for all applicable state sales and use taxes. Contractual Authority No written or oral statements of any person whosoever other than the Contracting Officer will, in any manner or degree, modify or otherwise affect the terms of this contract. The Contracting Officer is the only person authorized to approve changes in any of the requirements under this contract, and notwithstanding any provisions contained elsewhere in this contract, the said authority remains solely with the Contracting Officer. Submission of Invoices: The Contractor shall submit invoices to the Nellie.u.frith@usace.army.mil and Stacey.e.english@usace.army.mil Paper copies of invoices may be mailed to the following address: U.S. Army Engineering and Support Center, Huntsville, Attn: CEHR-ULC-EC (Stacey English), P.O. Box 1600 Huntsville, AL 35807-4301, For CLIN 0003 and during the option year periods, travel costs will be reimbursed in accordance with the Joint Travel Regulations (JTR). Travel expenses will be reimbursed at cost in accordance with the Joint Travel Regulation. Documentation of travel costs (i.e. hotel receipts, airline tickets, car rental receipts, gas receipts, etc.) shall be furnished with the invoice for which reimbursement is being requested. NOTE: Invoices must match purchase order/contract in description. Defective/deficient Invoices will be returned. Format for Invoices. Invoices submitted for payment by Contractors shall contain the following minimum information: (see FAR Clause 52.212-4(g) (1) Name and address of the contractor, (2) Invoice date and number, (3) Contract number, contract line item number and if applicable, the order number, (4) Description, quantity, unit of measure, unit price and extended price of the item delivered, (5) Shipping and payment terms, (6) Terms of any discount for prompt payment offered, (7) Name and address of official to whom payment will be sent, Payments will be made on a monthly basis after submission of a proper invoice and after the Government accepts the services. Contract Period. Any contract awarded as a result of offers submitted under this Request for Quotes shall extend from 20 October 2008, or date of contract award, whichever is later, through 19 October 2009, unless sooner terminated under the terms of the contract. Note: Exercise of the Government's option(s) to extend this contract may extend the above 19 October 2009 to as late as 19 October 2012. Point of Contact. The Contractor shall provide a local telephone number, or a long distance telephone number with reverse charges, that is answered 24 hours a day, seven (7) days a week, and a local address where the Contractor can be notified of or contacted for emergency work. The Government shall not be expected to call a series of phone numbers in order to locate the Contractor or his designated representative. Electronic answering devices, beepers, or pagers shall suffice for this requirement. Regulations. The site of some of the contracted work is on a Government facility, and rules and regulations issued by the Commander, U.S. Army Engineering and Support Center, Huntsville, AL, covering fire, safety, sanitation, severe weather requirements, admission to the facility and conduct of operations shall be observed by the Contractor and his employees.
