- The V A Pittsburgh Healthcare System intends to negotiate, on a sole source basis in accordance with FAR 6.302-1, with Landscape Forms Inc, 431 Lawndale Avenue, Kalamazoo, MI 4948, to purchase the following:CR999-06042-MMA,CAROUSEL 3 SEAT UNIT, BACKED, SOLID NSERT, SURFACE MOUNT, MARNEAUX TOP, NO UMBRELLA, 3 EA ; R999-06041-MMA, CAROUSEL 4 SEAT UNIT, BACKED, SOLID NSERT, SURFACE MOUNT, MARNEAUX TOP, NO UMBRELLA,5 EA; CR999-06040-MMA, CAROUSEL 5 SEAT UNIT, BACKED, SOLID INSERT, SURFACE MOUNT, MARNEAUX TOP, NO UMBRELLA 9 EA. These are custom made items. This Notice of Intent is NOT a request for competitive proposals.
