- Dental Services The Benjamin L. Hooks Job Corps Center, an adolescent training facility, is accepting proposals for the following contracted positions for the period of October 1, 2008 to September 30, 2010 with three 1- year options. Individuals or a Dental Group may submit a proposal. Bidders must possess current state of Tennessee license and malpractice insurance. Resume’, DEA number, and malpractice insurance required. Dentist – must be able to work a minimum of eight (12) hours per week. Dental Hygienist - must be able to work a minimum of eight (12) hours per week. Dental Assistant - must be able to work a minimum of eighteen (18) hours per week. Proposal must contain desired hourly rate for contract period and option years,. a copy of license, malpractice insurance, resume’, and DEA number (dentist only). Proposal must be received by September 26, 2008. Contract specifications may be printed from the attached file or obtained by contacting the Purchasing Department at 396-2800, ext. 2356.
