- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is solicitation number W91YTZ-08-T-0120 and is a request for quotation (RFQ). This requirement is 100% Small Business Set Aside. The Standard Industrial Classification Code is 6540 and the NAICS code of 339115. Womack Army Medical Center, Fort Bragg, North Carolina, 28310, intends to issue a contract for the following: ITEM 0001 Quantity: 1 Each Zeiss #VISANTE OCT 2.0 Optical Scanning Laser with software modules, table and printer, 2 operator manuals, 2 service manuals, installation and training with one year warranty on all products and two year warranty on the laser UNIT PRICE $__________________________ TOTAL $____________________ Delivery required: 30 - 45 days after receipt of order Responses can be emailed to Sandra.bulls@amedd.army.mil or faxed to Sandra Bulls @ (910) 907-9307, not later than 4:00 pm EST 23 September 2008.
