Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as a Request for Quotation (RFQ) No, FA7012-08-T-0072. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26. This is set aside 100% for small business concerns. The North American Industry Classification System code (NAICS) is 238990, with a business size standard of $13.0M. CONTRACT LINE ITEMS: 0001: Reception desk counter. Size: large. Design and construct accordance with the attached statement of work. QTY: 1 LS, Unit Price___________, Extended Price______________. 0002: Reception desk counter. Size: small. Design and construct in accordance with the attached statement of work. QTY: 1 LS, Unit Price______________, Extended Price_______________. STATEMENT OF WORK CROSSFIT RECEPTION DESK/COUNTER 1. DESCRIPTION OF SERVICES. The contractor shall furnish all personnel, management, tools, supplies, equipment, parts, materials, and transportation necessary to install, construct, and remove two reception desks/counters located at bldg P15 and P38, Fitness Centers on Bolling Air Force Base in accordance with the statement of work. 1.1. The contractor shall construct and install a large reception desk in The Fitness Center (P-15) and a smaller reception desk in The Aerobic Center (P-38). Contractor shall provide labor to disassemble existing reception desk, dispose, clean and prepare area's for installation of new reception desks/counter. 1.1.1. The contractor shall coordinate with the Fitness Center Director the date and time for the installation, construction, and removal of reception desks. 1.2. Large Reception Desk/Counter #1: The contractor shall disassemble and properly dispose of the existing reception desk/counter. The Contractor shall construct and install a large reception counter in the Fitness Center Building #P-15. The reception/desk counter size will measure 46 linear feet by 28 inches in depth. The staff work area (open space) within the reception desk/counter will have a dimension of 135 inches x 60 inches. The staff writing area front portion of the reception desk will have a height of 42 inches and will require space for a computer monitor. Cabinets with shelves shall be installed underneath the writing area. Doors shall not be installed on the cabinets to accommodate room for towel storage and a computer hard drive. The back portion of the reception desk shall have a counter top area for folding towels. Directly under this counter top there shall be 7 drawers with 7 cabinets. Each cabinet will have doors and contain one shelf. The front right portion of the counter top shall have an 18 inch x 18 inch square opening that shall serve as a towel drop. Directly underneath the towel drop there shall be a large cabinet 40.5 inches height x 32 inches length x 35 inches width with double doors to accommodate a towel cart. The floor of the towel cabinet will have a beveled front edge to allow the towel cart to roll into the cabinet. The front portion of the reception desk shall have an additional height of 11 inches x 8 inches depth, a total of 53 inches and serve as a privacy divider. This privacy divider shall contain lights for staff reading/writing. The reception/counter shall be constructed of wood laminate light grey with royal blue trim to match the Fitness center interior walls. Entry/Exit into the reception staff work area shall be located at the back portion of the counter. 1.3. Small Reception Desk/Counter #2: The Aerobic Center reception desk (P-38) shall have a dimension of 12 linear feet by 28 inches depth. The staff writing area front portion of the reception desk will have a height of 42 inches and will require space for a computer monitor. The front portion of the reception desk will have a height of 11 inches by 8 inches deep and serve as a privacy divider. This privacy divider will contain lights for staff reading/writing. It will be constructed of wood laminate light grey with royal blue trim. The staff writing area front portion of the reception desk will have a total height of 53 inches. The writing area shall have space to accommodate a computer monitor. Cabinets with shelves shall be installed underneath the writing area. Doors shall not be installed on the cabinets to accommodate room for towel storage and a computer hard drive. The back portion of the reception desk counter will have a standalone cabinet against the wall with shelves and doors for towel storage with the dimensions of 1 foot x 4 feet x 6 feet. This standalone cabinet will be constructed of wood laminate light grey with royal blue trim. There shall be a second standalone cabinet with the dimensions of 2.6 feet x 1.9 feet x 2.8 feet and will serve as a towel drop. The top portion of the towel drop will have an 18 inch x 18 inch square opening on top. The laundry cabinet dimensions shall have double doors and the floor of the cabinet will have a beveled front edge to allow the towel cart to roll into the cabinet. This standalone towel drop cabinet will be constructed of wood laminate light grey with royal blue trim. 1.4. See attachments: 1. diagram for large reception desk/counters 2. Diagram for small reception desk/counters INSPECTION CONDITIONS AND TERMS: Items shall be priced FOB Destination. Please include any shipping charges. Delivery/Acceptance will be at 11 MSG/SVMP 130 MCCHORD ST BOLLING AFB DC 20032-0305. A site visit will be conducted on Tues 16 Sep 08 At 10:00 a.m. EST at the Fitness Center (bldg P15) and the Aerobic Center (bldg P38) located at Bolling AFB, DC. ATTENDEES MUST HAVE CURRENT DRIVERS LICENSE, INSURANCE AND REGISTRATION TO ENTER THE BASE. Offerors planning to attend the site visit must provide the names of each representative by facisimile to TSgt Rhonda Y. Coles at (202) 767-7887 or email: rhonda.coles@bolling.af.mil no later than 15 Sep 08 at 3:00 pm EST. Each offeror may send up to two (2) representatives. If there are any questions after the site visit regarding this solicitation they should be submitted IN WRITING (by email or fax) not later than 10:00 a.m. EST, 17 Sep 08 to allow an answer period prior to the proposal due date. (E-mail is the preferred method of communication). Offerors are not required but strongly encourage to attend the site visit. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and Addendum to FAR Clause 52.212-1 applies to this acquisition are as follow: 52.252-1 http://farsite.hill.af.mil, 52.252-5: (b) DFARS (Chapter 2), and 252.225-7000. EVALUATION PROCEDURES: The Government will select the best overall offer based on Price and descriptive literature. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforming to the solicitation; will be the most advantageous to the Government in accordance with the requirement. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. If additional information is required to support price reasonableness, each offeror may be requested to provide a hard copy of their price break-down. Non-conformance with this requirement may result in offeror's quote being determined unacceptable. FAR 52.212-3, "Offeror Representations and Certifications-Commercial Items" applies to this acquisition and offeror(s) must include, a completed copy with its offer. FAR 52.212-3 can be obtained from http://farsite.hill.af.mil or from http://orca.bpn.gov if registered in ORCA (online Representation and Certifications application). An offeror shall complete only paragraph (j) of this provision if the offeror has completed the annual representations and certificates electronically at ORCA. If an offeror has not completed the annual representations and certifications at the ORCA website, the offeror shall complete only paragraphs (b) through (i) of this provision.
