- THIS IS NOT A REQUEST FOR COMPETITIVE PROPOSALS. The Department of Veterans' Affairs, James A. Haley Veteran's Hospital (JAHVH) located in Tampa, FL intends to solicit and negotiate a contract for ophthalmic equipment with Lombart Instruments on a sole source basis under the authority of FAR 6.302-1. The Government needs name-brand items to meet its requirements. Lombart Instruments offers most of the equipment on its Federal Supply Schedule. The remaining open market items shall include: Refraction Desk Prewire Kit (manufacturer: Expeditor Systems, part number: 62401); Slit Lamp Switch Prewire Kit (manufacturer: Expeditor Systems, part number: SL PLATE&CABLE); Indirect Hanger for Refraction Desk (manufacturer: Expeditor Systems, part number: 606); Standard Opticokinetic Drum (manufacturer: Machinery Works, part number: RADL300); Stereo Fly Test (manufacturer: Stereo Optical, part number: SO-001); Vertical Prism Bar (manufacturer: Gulden, part number:11117); Horizontal Prism Bar (manufacturer: Gulden, part number:11116);Lorgnette Pinhole Occluder (manufacturer: MDI Inc, part number: 10015): Rosenbaum Pocket Vision Screener (manufacturer: Allegheny Plastics, part number: ROSEN); Lombart Perimeter Trial Lens Set with Plus Cylinders Only (manufacturer: Lianyungang Tiannuo Optical, part number: ECHO); Haag-Streit Short Observer Tube with Eyepiece (part number: 09007.7); Zeiss SL 120 Slit Lamp with Parallel Binoculars; Tray and Pin for Zeiss (manufacturer: Shed, part number: TRAY). The contractor shall be responsible for shipping, installing, and training personnel where necessary in the use of the instruments. Interested persons may identify their interest and capability to respond to the requirement. This notice of intent is not a request for competitive proposals. However, all information received by 12 p.m. EST, 18 September 2008, will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will be considered solely for the purpose of determining whether to conduct a competitive procurement in future acquisitions. NAICS 339115 with a size standard of 500 employees.
